Dear Mr. Shook:
On March 27, 2019, Paul DiMatteo of the Division of Water Resources (Division), Winston-Salem Regional Office
met with you to conduct a Compliance Evaluation Inspection of the above referenced Certificate of Coverage. The
inspection typically consists of a review of the permit and accompanying documentation to determine
compliance with permit conditions and an on -site inspection of the wastewater facilitie. The enclosed
inspection form notes the areas that were evaluated for the inspection, with notable findings outlines as
follows.
1. Unless conflicting information is discovered, semiannual monitoring should be conducted as required in Part
1.A(1) for air conditioner winterization activities which result in a discharge of chlorine -treated water to the
roof drain system and eventually to Peter's Creek. Samples should be taken during a representative
discharge event, before the water is commingled with any other waste or substances, such as storm water
or surface water, and be analyzed by a certified laboratory. If there was no discharge during a semiannual
period that should be documented and the records maintained. Please be advised that the Division may
request to review documentation required to be kept by the Permit to determine compliance with its
conditions at any time.

On Mach 25,`2019 I met with Dylan Shook to inspect this facility. Mr. Shook explained that he took over
as manager of these programs for the plant about a year ago. Neither of us were entirely clear what
discharge, the permit is meant to cover - I was not able to find records in our files. There seem to be 2
possibilities:""
1. (More likely) there are large air conditioning units on top of the building that use pools of
chlorine -treated water in the cooling process. The water in these units is static, except when
winterizing they'may drain the water onto the roof, which would follow the roof drains to stormwater
outfalls and then"to Peter's Creek.
2. (Less likely) boilers are used in the manufacturing process. However, the discharge from these
boilers enter a pretreatment unit and join the City of Winston Salem's collection system; it is covered
under the City's Pretreatment Program.
I advised Mr. Shook to complete the monitoring required under Part LA(1) for the air conditioner
winterization unless advised otherwise. Samples should be taken during a representative discharge
event, before the, water is commingled with any other waste or substances, such as stormwater or
surface water.
