visited your home on March 28th, 2019, to perform a compliance evaluation inspection of your home's
discharging single family wastewater treatment system. Details of that inspection are included on the
attached EPA Water Compliance Inspection Report.

Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office, visited
your home on March 28th, 2019, to perform a compliance evaluation inspection of your home's
discharging single family wastewater treatment system. Details of that inspection are included below.
1. You were not aware of the need to check and add chlorine as necessary. Mr. Boone informed you
that
you should use a wastewater grade chlorine formulated with at least 60 percent calcium hypochlorite.
Please see attached documentation for chlorine guidance and sources of supply.
2. You informed Mr. Boone that you had the septic tank pumped recently and intend to do so every
three years, which is sufficient and complies with permit requirements. Please remember the tank and
solid's level in the tank should be checked yearly and the tank should be pumped whenever the level of
solids reaches 1/3 of the tank's liquid level, or every five years, whichever is sooner.
3. You informed Mr. Boone that you hadn't tested the effluent from the system and that you were
unaware of the requirement and the location of the end of the pipe into the adjacent pond. Even looking
at the system drawing in the home's file, your effluent pipe could not be located. Obviously, you cannot
collect a sample if you don't know where the end of the pipe is. Still, you should make every attempt to
find the end of the pipe and then collect a sample of the effluent and have it tested by a NC certified
laboratory as required on page 3 of the general permit.
4. A review of our database shows that you are current on the annual bill of $60.
We have attached several pieces of documentation, to this letter to assist you in properly managing
your system and maintaining permit compliance. The attachments are:
NC General Wastewater Permit NCG550000 (whole thing is important but first four.pages are most
important);
2. Technical Bulletin for the NCG550000 general permit (explains permit in layman's terms);
3. Chlorine Guidance Document and Sources of Supply; and,
4. List of NC Certified Laboratories
Please be aware, you should do everything within your power to comply with the terms and conditions
of the NCG550000 permit. Failure to do so subjects you, the permittee, to civil penalty assessments of
up to $25,000 per day per violation. The NC DWR wishes to help you attain an maintain perpetual
permit compliance as quickly and efficiently as possible in order to protect the. environment and your
welfare as a citizen of NC. When assessing permit compliance we take into consideration all factors,
including those which may be beyond your control.
You should take the following actions:
1. Acquire the proper chlorine tablets and apply them as needed for proper disinfection of wastewater
effluent;
2. Inspect and measure the septic tank's solid's level annually. Have the septic tank pumped at least
every five years or whenever the solids level reaches 1/3 of the tanks liquid level;
4. Continue to pay the annual permit fee of $60;
5. Attempt to find the end of the discharge pipe into the adjacent pond and then maintain the
right-of-way to it for future access;
6. Acquire a sample of the effluent annually and have it analyzed in accordance with the requirements
listed on page 3 of the.permit;
7. Keep all records for the home and wastewater treatment system in one place and readily available
for review and inspection, including but not limited to, system schematics, tank pumping records,

laboratory records, maintenance and repair records, permitting documentation, etc;
8. If/when you ever sell the home, ensure the new owners are fully aware of the special type of
wastewater system the home uses and that it's not just a traditional septic system with a drainage
field; and,
9. If/when you ever sell the home, notify the NC DWR of the change of ownership by filling out and
submitting an ownership change form, which you can acquire from us or our website here:
