Aiken, PEGA Regional Engineer Land Quality Section Subject: Notice of Violation (NOV-2019-PC-0213) NPDES Stormwater General Permit NCG 170000 Meridian Specialty Yarn Group, Inc. - Valdese Plant Certificate of Coverage NCG170030 Burke County, North Carolina Dear Mr. Aiken: [ C C^fvi t APR 2 9 2019 L_tnCi C L'?firyr , Se U _., iron Fa=i ��viile In response to the Notice of Violation that was received on April 3, 2019, Meridian Specialty Yam Group has taken the following actions to correct the noted violations. Item #1: The NOV noted that the stormwater pollution prevention plan (SWPPP) was not complete and not adequately maintained. The permittee was directed to review and update the SWPPP as required in the General Permit. Response: Upon review of the SWPPP we found that the site map and the stormwater discharge monitoring report for the last half of 2018 were missing. The site plan and monitoring report had been completed but apparently were not placed in the book containing the SWPPP. Meridian Specialty Yarn Group, Inc. has reviewed and updated the outfall inspections as required. The outfall inspections have been completed for the first half of 2019 and recorded in the SWPPP book. The plan has been reviewed and inspections will be recorded and maintained in accordance with permit requirements. An updated site map has been added to the book indicating the layout of the present building and property. The locations of Outfall #1, #2 and new outfall #3 (see item #5 below) have been recorded on the site map. Item #2: Noted in the summary of the facility inspection was the observance of bulk liquid storage without secondary containment. Response: The outdoor storage of bulk liquids without secondary containment has been addressed by Meridian Specialty Yam Group, Inc. All bulk liquids will be stored inside or within secondary containment in the fixture. The containers in question were unloaded and set outside temporarily before being placed inside of the building. The building is under major construction. Corrective actions have been implemented to rectify this situation. Item 43: The report noted that the secondary containment located at the southwest corner of the facility was damaged and had large cracks in the outer wall. Response: The secondary containment at the southwest corner of the facility was installed in relation to a retired #2 fuel oil storage tank. The 42 fuel oil storage tank is not in use and contains only a small amount of fuel. Immediate plans have been made to remove the small amount of fuel oil remaining in the tank. The tank and related secondary containment are being considered for removal. The boiler previously serviced with 92 fuel oil is now fired with natural gas. Item 44: Noted in the facility inspection was that access to the facility's outfalls had not been maintained. Response: Over time, vegetation has increased in the area near Outfall 41. The property near Outfall #3 will be improved with the completion of facility construction. Plans have been made and action will be taken to improve access to each of the facility outfalls for future monitoring events. APR 2 9 2019 Item 45: The facility inspection noted that an additional outfall was observed coming from the basin at the southeast corner of the facility. Response: The additional outfall resulted from the construction of an addition made to the existing facility. This outfall will be named Outfall #3 and has been added to the monitoring practices outlined in General Permit NCG170000. This new outfall (Outfall 43) has been added to the facility site map.
