Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The site is a road shoulder hillslope that slid in a February 2019 rain event. The area is rural and forested. The slide, which was blocking a LIT to Lands Creek, was removed and the road shoulder with rebuilt with shot rock. The LIT to Lands Creek was restored as close as possible to its original location using rock steps, coir matting, seeding, and planting of live stakes and bare root seedlings along the banks and in the buffer. 4b. Explain the purpose of the proposed project:* To repair a fill slope and rebuild a stream that was filled with slide material and fallen trees. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* Using heavy equipment (Track -hoes, dump trucks) the material that had slide from below SR 1364 was removed and the shoulder rebuilt with shot rock. A pump -around was used to avoid excavation in flowing water. To avoid armoring the entire stream, rock step pools were constructed near the pre-existing stream channel and ooir matting, seed, and livestakes/bare root seedlings were used to "restore" a more natural channel and buffer. 4j.
