However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on May 27, 2019. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy. Provide a copy of the engineer certification per the permit schedule. The certification provided in the additional information submittal was for WQ0037078 (sewer extension permit). The certification shall cover the following: a. 420 linear feet of 4-inch force main from the property line where the force main connects to the conveyance line from Bogue Watch WWTF to the Cannonsgate flow equalization basin b. Two 138 GPM pumps in the effluent pump station to convey treated effluent from Cannonsgate to Bogue Watch c. 173 linear feet of 4-inch force main from the effluent pump station to the property line where the force main connects to the dry conveyance line d. 4,839 linear feet of 4-inch ductile iron pipe from the Cannonsgate property line to the Bogue Watch property line 2. Provide an easement allowing Aqua to treat, store, and dispose of wastewater on all affected properties. The Groundwater Discharge Easement Agreement is only for groundwater and does not apply. Per 15A NCAC 02L .0107(f), an easement shall be executed and filed in the Register of Deeds in the county in which the land is located, an easement running with the land which: a. Contains notice of the permit, including the permit number, a description of the type of permit, and the name, address and telephone number of the permitting agency or a reference to a notice of the permit with book and page number of its recordation. b. Prohibits the construction and operation of water supply wells within the compliance boundary. c. Reserves the right to the permittee and the State to enter on such property within the compliance boundary for groundwater monitoring and remediation purposes.
