On January 8,2019,Paul DiMatteo and Kelli Park of the Division of Water Resources(Division),Winston-Salem
Regional Office met with Samuel Pegram, Operator in Responsible Charge (ORC) to conduct a Compliance Evaluation
Inspection (hereafter referred to as the initial inspection) of the above referenced wastewater treatment plant. An
unannounced follow-up inspection was conducted by Mr. DiMatteo,Ms.Park,and Lon Snider on February 6,2019.
This inspection consists of(1)a review of the permit and accompanying documentation to determine compliance
with permit conditions,and(2)an on-site inspection of several collection system components.The attached inspection form
notes the areas that were evaluated for the inspection,with notable findings outlined as follows. Where relevant,this report
also contains comments to your response to the previous Notice of Deficiency at this facility,NOD-2018-PC-0296,which
was received on October 29,2018.
Summary of Noncompliance
1. Discharge of visible solids in other than trace amounts.
Part I,Section A.1 and Part I,Section A.2 both require that,in addition to the numerical limits placed on the effluent,
"there shall be no discharge of floating solids or visible foam in other than trace amounts." During the initial
inspection, a significant amount of solids were observed leaving the ultraviolet disinfection chamber. An aliquot
collected at the effluent sampler appeared very turbid,as shown in Figure 1.
2. Proper Operation and Maintenance.
Part II,Section C.2 requires, in part,that"the Permittee shall at all times properly operate and maintain all facilities
and systems of treatment and control(and related appurtenances) which are installed or used by the Permittee to
achieve compliance with the conditions of this permit."The following operational problems were noted during the
inspections and require immediate attention. See also the included pictures.
a. The foam in the northwestern train's aeration basin covered more than 50% of the basin during both
inspections.
b. Floating sludge was observed in both clarifiers during both inspections.
c. During the follow-up inspection, a thick layer of sludge covered the northwestern train's clarifier. Sludge
was observed flowing over the clarifier weirs and accumulating in the trough. Mr. Pegram said that he
thought there had been a1filamentous bacterial bloom,and that he planned to kill the bacteria with chlorine

and pump off the solids. Please note that any addition of chlorine during the treatment process will also
require effluent residual chlorine analysis.
3. Composite/Representative Sampling.
The previous Notice of Deficiency noted that the effluent composite sampler must be properly set for flow
proportional sampling. The response indicated that"the sampler has been set to flow proportional sampling."
During the initial inspection,the composite sampler appeared to be set to collect a 425 ml aliquot every 1 pulse,
with each pulse representing 600 gallons, and at most 96 aliquots per sampling event. The sample jug held
approximately 3 gallons. Using the average daily flow of 58,000 gallons,the sampler would thus pull 96 aliquots,
totaling 40.8 liters of sample throughout the day.This would appear to lead to an overflow of the sample jug after
only about 6 hours of sampler run time.
During the follow-up inspection,Mr. Pegram said that there was an issue with the composite sampler a few weeks
ago and he called another operator to help him set it up again. It was set to take an 80 ml aliquot every 3 pulses,
with each pulse being 600 gallons,and a maximum of 50 aliquots per sampling event. This alleviates many of the
issues identified in the initial inspection.However,it should be noted that each aliquot taken for compositing must
be at least 100 ml in volume.
Please note that Part II, Section D.1 requires that samples and measurement taken as required by the Permit"shall
be representative of the permitted discharge."Where composite effluent sampling is required,the Permit defines a
Composite Sample as "A sample collected over a 29-hour period by continuous sampling or combining grab
samples of at least 100 ml in such a manner as to result in a total sample representative of the wastewater discharge
during the sample period."(emphasis added).Care must be taken to ensure that these samples are representative of
the entire discharge for that period. Operators should be familiar with the operation of the composite sampler and
flow totalizer interface so that they are able to make adjustments when necessary(for example,it may be necessary
to increase each pulse volume to ensure that the sampler runs throughout the course of a 24 hour period when higher
than average flow is expected,or that at least 24 aliquots are obtained when lower than average flow is expected).
Other Items of Concern
• 4. Flow Measurement
The previous Notice of Deficiency indicated that the effluent flow meter appeared to be reading below the 10%
compliance threshold.The response indicated"We are having the calibration verified by a third-party vendor."
During the initial inspection,Mr.Pegram produced a flow meter calibration dated October 30,2018 by ClearWater,
Inc., in which the calibrator appeared to further adjust the meter's calibration down by 0.12 feet. When checked,
the head at the weir measured about 0.27 feet(table value 42.5 gpm), but the totalizer reported about 23 gpm, far
below the expected value.The inspector further noted that the monthly average flow in November 2018 was about
62%of the average monthly flow for the period 1/2016 through 10/2018, and about 88% of the monthly average
flow for the next-lowest month,which occurred in July 2018.
During the follow-up inspection, Mr. Pegram said that someone had since looked at the flow meter again. A
calibration report was not yet available. A measurement taken at the effluent weir measured about 0.30 ft(table
value 55.31 gpm); the flow meter reported about 60 gpm. The calibration now appears acceptable. Please ensure
that the flow measuring devices are at all times "maintained to ensure that the accuracy of the measurement is
consistent with the accepted capability of that type of device"(see Part II,Section D.3 of the Permit).
5. Standby Power
Power to the plant was interrupted during the initial inspection, but the standby generator did not come on. Mr.
Pegram contacted a vendor and requested that they evaluate the generator.When tested again during the follow-up
inspection,the generator came on and was capable of powering the entire plant.

This Notice of Violation has been issued for the violations noted above.Please note that,pursuant to North Carolina
General Statute (G.S.) 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be
assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any
permit issued pursuant to G.S. 143-215.1. Your immediate attention is correcting these items is appreciated.If you wish to
provide additional information regarding the noted violations please submit a written response for review within fifteen(15)
business days after receipt.

Comment: Effluent composite sampler was set to sample every pulse,with a maximum of 96 samples
in a 24 hour period. One pulse was 600 gallons.An aliquot pulled at the effluent sampler
measured--425 ml. 96 samples*600 gallons per sample=>the sampler will finish after
57.600 gallons. It should be noted that the monthly average reported flow for this facility
exceeded 57,600 gallons in 23 out of 36 of the months from 1/2016 through 11/2018.Thus,
the sampler will finish in less than 24 hours on the average case.Additionally. 96 aliquots*
425 ml/aliquot=40,800 ml(10.7 gallons) of sample volume collected, but the sample
container only held about 3 gallons. Mr Peqram said that the sample jug was often very
close to full when the sample was collected the next day.

An unannounced follow up inspection was conducted on February 6. I informed the ORC of the
impending inspection at approximately 9:00 am and arrived at the facility at 9:30 am. I was joined by
Kelli Park and Lon Snider. On arrival, a man pulled up who said he was a contractor for Aqua working
on a panel in the back(it looked like the electrical panel for the effluent pump station was replaced). He
left the gate to the plant unlocked but we did not enter.We walked down to the effluent pipe, which was
still under water but not as much as during the last inspection,and then waited outside the plant until
the operator arrived at approximately 10:30 am.
The primary purpose of the inspection was to get an update on the condition of the plant,verify whether
or not the effluent sample bottle had overflowed,and verify the flow meter's calibration again.
There was a thick layer of sludge floating on top of the clarifier, and had even accumulated after the
overflow weirs. Mr. Pegram said that filamentous bacteria cropped up over the weekend. He said he
planned to use chlorine to kill them and add some dechlor tablets.
Mr. Pegram said that someone with ClearWater was out on Monday to again check the flow meter. I
measured approximately 0.30 feet of head at the weir(table value 55.31 gpm)-flow totalizer measured
approximately 60 gallons.This appears acceptable.
Mr. Pegram also said that there was a problem with the sampler a week or so ago and he called a
fellow operator and had him help set up the sampler. It appeared to be set to sample 80 ml every 3
pulses,with a maximum of 50 aliquots per sampling event.The jug appeared to have about 1 gallon of
sample or less in It.This set up appears acceptable,with the exception that the aliquot must be at least
100 ml in volume.
Power was interrupted to the plant and the backup generator came on and was capable of running the
plant. •
