Division of Water Resources' Central and Regional staff has reviewed the application package
received February 6, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 24,
2019.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy.

1. The previous permit indicates one 1.3 million gallon aerated treatment lagoon and one holding pond. Provide the
approximate capacity of the holding pond. Also, clarify if any of the other lagoons or structures on site are part of
the non -discharge facility.
2. The groundwater monitoring data is showing pH outside of the 15A NCAC 02L groundwater standards range.
Please provide the following to help determine the source of the groundwater acidity:
a. Past 5 years of soils analysis
b. Field maintenance records (lime and fertilizer applications)
c. Irrigation field crop(s)
3. The irrigation fields have only 2 monitoring wells within the compliance boundary and no wells on the review
boundary. Considering the irrigation area covers 130 acres, it would be in the permittee's best interest to install
additional monitoring wells on the review boundary. Please address.
