Explanation.
Team Industries was found to be in SNC during 2018. For the period of January —June 2018, Team
Industries was in SNC for BOD and Cyanide. For the period of July — December 2017, Team Industries
was in SNC for Aluminum.
On January 31, 2019 Brian Gant provided the results of all compliance testing Team Industries had
performed through its contract lab through 2018.
On September 12, 2018 the Town of Andrews issued NOVs to Team Industries, Inc. Anchor QEA on
behalf of Team Industries indicated that since the process water for the treatment had been reduced
greatly, the effluent had greater concentrations of pollutants and asked if the Town of Andrews would
consider raising their effluent limits.
On November 14, 2018 the Town of Andrews approved the proposed increase of permit limits for Team
Industries raising the BOD limit to 1400 mg/L; Aluminum to 15.0 mg/L; Cadmium to 0.070 mg/L;
Chromium to 1.71 mg/L; Cyanide to 0.650 mg/L; Copper to 2.07 mg/L; Lead to 0.430 mg/L, Zinc to 1.470
mg/L; Silver to 0.240 mg/L. A new permit was issued to Team Industries.
Sampling was performed for the Town of Andrews by Earth Environmental Services on 11/27 & 28/18
and then again on 12/11 & 12/18.
On April 3, 2019 Anchor QEA responded to the NOVs. The night prior to the sampling events, the gas
that fed the heater in the facility's treatment area was mistakenly shut off causing temperatures to drop
significantly. This caused the aluminum in the bottom of the settling tank to roll to the top of the tank
causing excesses in TSS and Aluminum during that time. Team subsequently restored the gas to the
heaters and the wastewater treatment was returned to normal.
Automatic ignition systems were added to both heaters in the treatment area to prevent this problem in
the future. Additionally, a wastewater treatment specialist with Anchor QEA is assessing the inputs and
treatment system in an effort to improve effluent quality.
On March 25, 2019, the Town of Andrews did a pretreatment inspection of Team Industries and found
the plant and processes to be in good shape. Earth Environmental Services performed sampling on

March 26 and 27, 2019 and BOD & TSS from that sampling event were well within compliance. Other
results from that sampling event are pending.
If Team Industries is not back compliance in 2019, the Town of Andrews is considering levying additional
sampling requirements on Team Industries.
Team Industries, The Town of Andrews WWTP Personnel, Anchor QEA and Earth Environmental Services
are working together to determine what further steps need to be taken to bring all pollutants beneath
permit limits and to bring Team Industries Andrews, Inc. back into compliance.

Team Industries Andrews, Inc. has been found to be in violation of Part I., for their Wastewater
Discharge Permit (#0004) issued by the Town of Andrews for the Determination Period of 111118
— 6/30118. The violations are summarized on the following page. Please note that the industry is
in Significant Non -Compliance for BOD and Cyanide.
A report containing the probable cause of the violations and plans to return to compliance must
be submitted to this office within 30 days of receipt of this notice. Team Industries Andrews, Inc.
will be included in the Public Notice of Significant Non -Compliance as required by NCAC 02H
.0908 and 40 CFR 403.8 (f).
If Team Industries fails to return to compliance, then additional actions, including increased
penalties or issuance of an enforceable order, will be completed in accordance with the City's
Enforcement Response Plan. Please be aware that the City's Sewer Use Ordinance gives the
Director the authority to assess penalties of up to $25,000 per day per violation.
