I am writing to you in regards to the violations that were present on my farm on Septem`�°��

2018.
Violation 1:
In the letter our lagoon, levels were documented at 6, 17, and 17. Our levels were documented
at these levels because of the massive rainfall present during Hurricane Florence. Our farm has
irrigated to ensure that our lagoon levels remain under a level of 19.3 inches which is stated in
our permit.
Violation 2:
During'this-tirime period; our.farrim'had hired -an individual to keep all of our irrigation records and
to turn in Plan' of'Actions,'etc. 'Therefore'we had the uridersfanding'that fhis'was 'already -
completed. This is my fault for not making sure it was complete. To correct our mistake we plan
to send a Plan of Action for the high freeboard from the date of September 26th, 2018.
Violation 3:
Due to the tree limb growing through our recycle lagoon (i.e. #3) we will approach this problem
under the supervision of Glenn Clifton of Prestage Farms LLC.
Violation 4:
During the time the irrigating, the reel was stationary because of the fact that we had just turned
the irrigation system'on and our serviceman was onsite. The irrigation pipes are all being
replaced'dub.to'the leaking from these' pipes. • - . - ' '
Violation 5:
The September 26th irrigation event was not properly recorded on an IRR-2 form dh tod4ur9���
hired record keeper having all of the records and forms. This is my fault for not being prkNap 9

and having the appropriate forms in hand. This will be handled for future events by keeping df,
records on the farm.
Violation 6:
As stated above we were paying a record keeper to maintain all of the records and they failed to
do so. This is my fault; I should have done this myself to ensure it was done correctly. An
employee of Prestage Farm LLC will be conducting our sludge survey and it will be sent in
immediately
Violation 7:
Our record keeper was given the crop yield records due also failed to record this as well. This is
my fault for not keeping up with our record keeper to make sure he was doing what he was
supposed to do. This will be completed on the appropriate form and sent in immediately.
Violation 8:
In accordance with our Certified Animal Waste Mangement Plan, we failed to pay our permit
fees for 2018 and for 2019. This is in the process of being taken care of and will be sent to the
Animal Feeding Operations Program in Raleigh, North Carolina.
I have always taken hog farming and the rules and regulations that are required in my permit
seriously. I try my best to be a good environmental stewardess when irrigating and doing other
routine jobs around my farm. I do everything in my power to prevent any environmental
problems from occurring, but nothing is ever perfect and I have made a mistake. From now on I
will try my best to prevent this from happening again, and if any problems do occur again I will
