TOWN OF MAIDEN WWTP NPDES PERMIT # NCO039594 2018 PAR NARRATIVE The following is a narrative of the Town of Maiden Pretreatment Program for 2018 (January — August). The narrative outlines general information about the Town of Maiden Pretreatment Program. The narrative also outlines the performance of the Town of Maiden's permitted Significant Industrial Users and Categorical Industrial Users for 2018 and actions, if any, taken against them. General Information. General Program Information` • AT, HWA, STMP, SUO, ERP, IWS, SIU Permits, and Industrial inspections are up to date. • Tim R. Hedrick is currently the Town's Pretreatment Coordinator • All industrial annual inspections were completed • A single NOV was issued to the only CIU/SIU • Maiden still only has the one CIU/SIU (GKN Driveline) • There are no active CO/SOCs • GKN permit was renewed in 2017 • GKN has been issued a single NOV for Copper violation on 03/23/2018 and is also in SNC for the first six month period. • A copy of our Program info sheet is enclosed General Permit Information • GKN Driveline is still the Town of Maiden's only CIU/SIU. • GKN Driveline had a violation for Copper on March 23, 2018. A single NOV was issued and also GKN was put into SNC for the first six month period • The GKN Driveline permit is valid until December 31, 2021. SIU and CIU Program Information IUP # 001 GKN Driveline (40 CFR 433) GKN Driveline is still the only permitted CIU/SIU. Narrative — PAR 2018 Town of Maiden WWTP GKN stopped discharge to the Town of Maiden on August 22 2018 to dismantle there treatment process and to start construction on their new EVAP treatment process going forward in the future. This CIU was in Significant Non -Compliance for the first six-month period for 2018. GKN's Pretreatment System continues to produce a high quality effluent that is discharged to the town's collection system. During the first six-month period of 2018, one violations occurred. During the first six-month period of 2018, GKN Driveline exceeded the permitted Copper limit on March 23 , 2018. A notice of Violation has been sent to GKN. And also they are in SNC for the first six month period Waste Reduction Activities The Town of Maiden continues participation in curbside recycling for plastics, glass, aluminum, metal cans, water heaters, and newspaper. The Town will also pickup and properly dispose of TV's, and offers drop boxes at the police station for pharmaceutical disposal. In addition to the Town's recycling efforts, the industries participate in several waste reduction activities. GKN Driveline participates in the following waste reduction activities: W13, W21, W24, W31, W34, W36, W54, W55 and W68. Tim R. Main Ave • Maiden, NC 28650 Office (828) 428-5032 • Fax (828) 428-5606 November 07 2018 Subject" Request to drop Industrial Pretreatment Permit Permit IUP #001 40 CFR Category 433 GKN Driveline Maiden, N.C. 1, am writing in response, to Industrial Pretreatment User, (IUP) #00140 CFR Category 433 GKN Driveline, To request to drop there permit from the Town of Maiden, Pre Treatment Program and To also put the Town of Maiden Pre Treatment Program to inactive status. On 8/22/2018 1 received A email from Mr. Lawrence Kearson stating that GKN had stopped discharge from the pre treatment Facilities located at 1848 GKN Way to the Town of Maiden Waste Water Treatment Plant. The stoppage is in preparation of the dismantling of the current system, and the installation of the new Evaporation unit that will treat the wastewater going forward. In regards Mr. Kearson also stated that they would be no quarterly sampling for the current quarter as required by permit, and the flow log will show discharge through 8/21/2018. 1 also requested him to continue to send me flow logs to keep On file and records through this process. Upon completion of this project, I have went back and done a Final walk through and inspection to assure that everything is accurate, and one hundred percent to my satisfactory. I see no for seen reason why GKN needs to be on a ]UP permit for the Town of Maiden At this point and time, and I highly recommend and request to drop there permit status.
