POST OFFICE BOX 1396 OXFORD, NORTH CAROLINA 27565 Dear Mr. Smith: NORTH CAROLINA Environmental Quality April 17, 2019 Subject: Permit No. WQ0035595 Granville Farms RLAP Attachment A and B Certifications Edgecombe County In accordance with your permit major modification request received March 21, 2019, we are forwarding herewith modified Attachments A and B certified April 17, 2019, to Granville Farms, Inc. for the subject residuals management program. Modifications to the subject Attachments are as follows: 1. Addition of three new residuals sources totaling 1,436 dry tons/year (dt/yr), those sources being the Sunset Avenue WTP (Permit No. NC0072133, 318 dt/yr), Tar River Regional WWTP (Permit No. NC0030317, 800 dt/yr) and the Tar River WTP (Permit No. NC0072125, 318 dt/yr). 2. Increase in one residuals source from 500 to 1,000 dry tons/year, that source being the Wilson WWTP (NC0023906). 3. Transfer of 17 fields in Edgecombe County from Permit No. WQ0002047 totaling 152.89 acres, those fields being TI-01 to -10 and T6-01 to -07. The modified Attachments A and B shall be effective from the date of certification until September 30, 2021 (i.e., expiration date of the current permit), shall void Attachments A and B of Permit No. WQ0035595 issued October 10, 2016, that were previously certified on September 17, 2018, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above - referenced permit. Please replace Attachments A and B of Permit No. WQ003 5595 that was issued October 10, 2016, with the modified Attachments A and B. Please note that the Permittee shall submit a renewal application no later than April 3, 2021. Please note that only the maps for new land application sites, Figures 9 to 11, are attached to this letter.
