Dear Mr. Weaver:
Division of Water Resources' Central and Regional staff has reviewed the application package
received February 28, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 16,
2019.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through D, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Soil Evaluation:
1. Only one soil boring description was submitted inside the proposed waste irrigation area. However,
the Division's regional soil scientist indicated that this description does not appear to describe other
parts of the proposed irrigation area. Additional soil boring logs shall be performed within the
proposed waste irrigation area to verify the field description and depth to the Seasonal High Water
Table (SHWT).
2. The full extent of the proposed irrigation field was not able to be determined by the regional soil
scientist because the area was not flagged. Mark the boundaries of the irrigation field so that the
regional soil scientist can inspect the field.
3. The proposed field location is directly adjacent to the 100-year flood zone. Due to the proximity of
the field to this zone, the extent of the Flood Hazard Soils should be determined in the field with
respect to the proposed irrigation area to determine if the proposed irrigation area has adequate
separation to the SHWT in accordance with 15A NCAC 02T .0605(p)-(q).
B. Engineering Plans:
1. The property layout map on WW-2 includes a 50' stream buffer, but the setback to surface waters
such as intermittent and perennial streams is 100' according to 15A NCAC 02T .0606(a). Please
amend this buffer.
2. The property layout map on WW-2 does not include the location of wetlands in the northeast of the
property as acquired from National Wetlands Inventory data as shown on this map. Please add this
feature to the map as well as the 100 foot buffer as required by 15A NCAC 02T .0606(a).
C. Site Map:
1. See B.1. and B.2.
D. Recommendations:
Field work that should be performed prior to Division's soil scientists second visit include
additional soil borings in the proposed field and delineation of all proposed facilities. These
facilities include the house location, the treatment tanks, and the irrigation site.
The regional soil scientist indicated that there are potential wetland areas on the site. These wetlands
were potentially within the irrigation area, though the location and extent of the feature is unknown.
If there are any areas inside or within 100 feet of the proposed irrigation area that could be
considered a wetland are noticed while flagging the irrigation area, a wetland determination is
recommended. If inspection the Division finds evidence of wetlands within these areas during the
second site inspection, a wetland determination may be required.
3. We recommend that consultants meet with Division staff at the site to discuss the application after
the additional field work has been completed.
