However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on May 15, 2019. Application (15A NCAC 02T .0105): 1. Provide the permit number(s) for the collection system and reclaimed water distribution lines. 2. The provided easement between Randy Blanton and Aqua references a plat map (Book 40, Page 32). Provide a copy of the plat. 3. It appears the irrigation fields are located on Castle Bay Corp. property. Provide an easement. Per 15A NCAC 02L .0107(f), an easement shall be executed and filed in the Register of Deeds in the county in which the land is located, an easement running with the land which: i. Contain notice of the permit, including the permit number, a description of the type of permit, and the name, address and telephone number of the permitting agency or a reference to a notice of the permit with book and page number of its recordation. ii. Prohibit the construction and operation of water supply wells within the compliance boundary. iii. Reserve the right to the permittee and the State to enter on such property within the compliance boundary for groundwater monitoring and remediation purposes. 4. To ensure the Division's records are correct, please verify the following in regards to the upset pond and effluent storage pond: a. Effective storage capacity b. Liner type c. Designed freeboard d. Approximate date built B. Site Map: The provided maps are incomplete. Provide a site map showing: 1. Distance measurements verifying all setbacks are being met (or minimum distance per setback waiver) 2. Stormwater drainage controls (ditches, stormwater drains, etc.) 3. Location and ownership of property boundaries within the disposal area (including road/rail right-of-ways within 100 feet) 4. All facility -related structures and fences within the treatment, storage, and disposal areas 5. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites 6. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s) 7. Location and identification of each monitoring well (identify any background/upgradient wells) 8. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits) 9. Location and identification of major components of the waste disposal system 10. Identification of all storage or treatment earthen structures that are part of the permit 11. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved permit) 12. 100-year floodplain (if applicable) Ms. Amanda Berger April 15, 2019 Page 3 of 3 C. Setbacks 1. Ensure all setback are met or have a setback waiver. The facility was originally permitted in 2001 and is required to meet the following setbacks: a. Irrigation sites: i. Surface waters classified SA: 100 feet ii. Surface waters not classified SA: 25 feet iii. Each water supply well: 100 feet iv. Each non -potable well: 10 feet v. Each swimming pool: 25 feet b. Treatment and storage facilities: i. Each habitable residence or place of assembly under 100 feet separate ownership: ii. Each private or public water supply source: 100 feet iii. Surface waters: 50 feet iv. Each well with exception of monitoring wells: 100 feet v. Each property line: 50 feet vi. Each on -property residential unit if the unit is to be sold: 10 feet vii. Nitrification field: 20 feet A Compliance 1. Per Tom Tharrington, the regional inspector, the irrigation equipment calibration records were not available at the time of inspection. Per 15A NCAC 02U .0801(h), the dates of calibration irrigation equipment must be kept in a maintenance log at this facility. Provide a status on the plan to install a flowmeter from the effluent storage pond to the golf course irrigation system. E. Groundwater 1. Provide location (same as requested for the site map), construction information, and completed depth for all monitoring wells. 2. The ammonia levels for MW-2 have been exceeding the 15A NCAC 02L groundwater standards and have steadily increased over time. Provide a groundwater analysis report to address these exceedances. Per 15A NCAC 02L .0108, when the concentration of any substance exceeds the standard at the review boundary, the permittee shall take action in accordance with the provisions of Rule .0106. The permittee is recommended to schedule a meeting/phone conference with the Central Office and Regional Office staff to discuss.
