on November 20, 2013. The facility was found to be in Compliance with permit WQ0002645.
Two issues noted during the inspection:
1. The Operation & Maintenance Plan for the land application program needs to be completed.
2. Dennis Key is the designated land application Operator in Responsible Charge (ORC). Please
confirm that he is still the primary ORC and if not, please submit an ORC Designation Form for
his replacement.

Inspection Summary:
The Town is in the middle of a WWTP upgrade. The WWTP will have a new 500,000 gallon sludge holding tank. The
ORC anticipates the next land application event will likely be in 2015. The plant upgrades are expected to be completed
in April 2014. Residuals are being held in the earthen basin adjacent to the aeration basin. The earthen basin is not
aerated. The supernate is being pumped back to the head of the WWTP. This process will continue until the new tank is
on-line. According to Paul Phodes and Rob Rosseter, there is enough capacity in the basin to contain residuals until the
new tank comes on-line.
The 2012 Annual Report appeared to be complete and accurate.
The Operation & Maintenance Plan needs to be developed for this facility.

Inspection Summary:
A new pasteurization vessel, auger screw and heating element was installed in October.
The residuals storage bin is almost full but Mr. Smith feels they still have enough room for storage as long as distributions
pick up within the next couple of months.
The records appear to be complete and accurate. The 2012 Annual Report was also reviewed as past of the inspection.
The facility is well maintained.

The Town has just started sending residuals to the Foothills Landfill located in Lenoir, NC. Two dumpster loads have
been hauled so far. GDS from Boone has been contracted to haul for the Town of Burnsville.
Hauling tickets were available for review. At this point no final decision has been made by the Town as to whether they
will continue to compost residuals.
The records appear to be complete and accurate.
Reminder: The metals/nutrient analysis is due before the end of the year.

the inspection conducted on October 14, 2013. The components of the system as listed
in the permit description appear to be in place and operational. ,The Engineer's
Certification has been submitted. This inspection satisfies the permit condition 1.2.
which requires an in-place inspection at least 48 hours prior to operation of the new
facilities.
Please submit a minor permit modification to update Attachment A & Attachment
B of the subject permit. This update will allow you to track total effluent distributions as
opposed to effluent distributions to each site. There is no fee associated with this
modification, nor is an application required. The request can consist of a one page
letter addressed to the attention of Mr. Cory Larsen at the following address:

Please find attached the analytical results for seven water samples collected from your well
located at the address referenced above on August 5 and October 21, 2013. The water samples
were collected in conjunction with the Macon County Environmental Health Department's lead
in private drinking water wells investigation.
` During the August 5, 2013 site visit,two water samples were collected at the well head. The first
sample was collected prior to purging any water from the well (i.e. "first draw") and one was
collected after purging 2 gallons from the well. The groundwater samples were submitted to the
laboratory for analysis of selected metals. The following table summarizes the volume purged
from the well prior to sample collection and the analytical results. The Environmental Protection
Agency (EPA)maximum contaminant level (MCL) or action level and the North Carolina
Administrative Code (NCAC)Title 15A 2L .0202 groundwater standards are included for
reference. The metal results are shown in units of micrograms per liter(µg/1) of water, which is
equivalent to parts per billion(ppb).

The groundwater samples collected during the August 5tn sampling contained concentrations of
lead in excess of the EPA's action level and the NCAC 2L groundwater standard and zinc in
excess of the NCAC 2L groundwater standard. I understand that in response to these ,
exceedances, and based on recommendations from this office, in late September to early October
you hired a licensed well contractor to replace the galvanized piping and tee at your wellhead
with a stainless steel tee.
I returned to your well on October 21, 2013, and collected additional groundwater samples at the
well head. During the October 21St site visit, approximately 60 gallons of water were purged
from the well over the course of 23 minutes. Five water samples were collected at the well head
over the course of the 23 minute purge and submitted to the laboratory for analysis of selected
metals. The following table summarizes the results from the October 21 st sampling.

No metals were detected above their respective EPA MCL or action levels or their NCAC 2L
groundwater standards in the samples collected during the October 21 st sampling.
The groundwater analytical results for the October 21St sampling were provided to Dr. Ken Rudo,
a toxicologist with the Occupational and Environmental Epidemiology Branch of the North
Carolina Department of Health and Human Services, for a Health Risk Evaluation(HRE). An
inorganic chemical analysis report completed by Dr. Rudo is attached. The report indicates that,
based on the results from the October 21 st sampling, your water can be used for drinking,
cooking, washing, cleaning,bathing, and showering. If you have any questions regarding the
report please contact Dr. Rudo at(919) 707-5911. Please note that the HRE assumes the use of
lead-free faucets in your home.

On December 16, 2013, staff of the NC Division of Water Resources (DWR), Water Quality Regional
Operations(WQRO),reviewed the Groundwater Quality Monitoring: Compliance Report Forms(GW-59
Forms)for the November 4,2013 monitoring at the subject facility.
The following items were determined to be missing at the time of the review:
1. Total Suspended Solids (TSS) results were not reported for monitoring wells MW-1R, MW-2,
MW-3, or MW-4. Please note that this parameter was added to the most recent version of your
permit,dated October 29,2013.
You are required to take corrective measures regarding Item 1 above on or before January 16, 2013. If
you collected the missing data during the November 2013 sampling, please re-submit the GW-59 forms
with the TSS data included. If you failed to collect a groundwater sample for analysis of TSS, please
make sure to collect these data during the May 2014 sampling.
Please send a letter to our office acknowledging receipt of this Notice of Deficiency and the corrective
measures you plan to make. Failure to comply with the State's rules and correct the noted deficiencies by
January 16, 2013, may result in the assessment of civil penalties and/or the use of other enforcement
mechanisms available to the State.

This letter is to follow up on my site visit with you on December 13, 2013. The site visit was a
result of a complaint that dry poultry litter is being stockpiled uncovered for longer than 15
days at your facility. Although the litter was covered at the time of the investigation,you
indicated that the tarps were only replaced on December 12th.
You will recall that we have responded to several complaints at this facility. I have provided
Johnny Newton with copies of our regulations (15A NCAC 02T .1303-Permitting by Regulation
& Manure Hauler Regulations 15A NCAC 02T.1400) pertaining to the management of dry
chicken litter. I have attached these same documents for your review.
15A NCAC 02T.1303(a)(2) requires that your operation not stockpile litter longer than 15 days
uncovered. According to the complaint, litter is regularly stockpiled without cover. Please note
that the entire litter pile is to be covered if it remains onsite.

Please find attached the analytical results for a water sample collected from your kitchen tap at
the address referenced above on November 18, 2013. The water sample was collected in
response to a recommendation from the Occupational and Environmental Epidemiology Branch
(OEEB)of the North Carolina Department of Health and Human Services to resample your water
following the detection of dieldrin at a concentration of 0.06 micrograms per liter (µg/1) in a
sample collected from your well on September 25, 2013.The results of the September 25, 2013
sampling were provided to you in a letter dated October 24, 2013. The recommendation to
resample was provided on a Health Risk Evaluation (HRE) completed by the OEEB, which was
provided to you by Mr. Landon Davidson of this office in an email on October 25, 2013. A copy
of the HRE is attached.
During the November 18, 2013 sampling, a water sample was collected for analysis of fecal
coliform bacteria,total coliform bacteria, and chlorinated pesticides. No fecal or total coliform
bacteria were detected in the sample. Dieldrin was again detected at a concentration of 0.06
jig/I. No other pesticides were detected in the November 18, 2013 sample.

Please find attached analytical results for soil samples collected from your property located at the
address referenced above, on July 22, 2013 and August 27, 2013.The soil samples were submitted
for laboratory analysis of chlorinated pesticides.The purpose of the soils investigation is to
determine whether groundwater contamination is related to the application of agricultural
pesticides or the use of building foundation termiticides.
The attached table (table 1) summarizes the analytical results.The results are shown in units of
micrograms of contaminant per kilogram of soil and are approximately equivalent to parts per
billion.The analytical results are compared with residential soil screening tables developed and
maintained by the United States Environmental Protection Agency(USEPA) and the North Carolina
Inactive Hazardous Sites Branch (NCIHSB).The USEPA soil screening table provides acceptable levels
of soil contamination not associated with adverse health risks.This table takes into account both
cancer and non-cancer risks as well as exposure pathways including ingestion, dermal (skin) contact,
and inhalation.The NCIHSB soil screening table provides acceptable levels of soil contamination
that are not expected to result in groundwater contamination.
Soil samples collected on July 22, 2013 focused on the backyard and next to the house foundation
(Photo 1).The backyard samples(SS1) included 4 boreholes aligned perpendicular to crop rows
visible on a 1951 aerial photo.A single borehole was used to characterize soils next to the house
foundation (SS2).Samples were collected at depth intervals of 0—6 inches and 15—21 inches. It is
important to note that the backyard samples were created by compositing material from each of
the four boreholes and reducing the volume into two duplicate soil samples. DDE and DDT isomers
were detected in the backyard samples but concentrations did not exceed the USEPA and NCIHSB

soil screening levels. DDE isomers, DDT isomers, and dieldrin were detected in soils adjacent to the
house foundation. Dieldrin was the only analyte to exceed the USEPA and NCIHSB soil screening
levels.
Samples (SS3)collected on August 27, 2013 focused solely on soils next to the house foundation
(Photo 1).A single borehole was used for the foundation sample.Samples were collected every 0.5
feet down to a maximum depth of 3 feet.Aldrin, DDD isomers, DDE isomers, DDT isomers, dieldrin,
endrin,endrin ketone, endosulfan sulfate, and toxaphene were detected next to the house
foundation. Dieldrin was the only analyte to exceed both the USEPA and NCIHSB soil screening
levels.Aldrin and toxaphene exceed the NCIHSB soil screening level.The highest concentrations
occur in the top 1 foot of soil.
The presence of aldrin and dieldrin and their proximity to your home's foundation suggest your
home was treated for termites using some combination of aldrin and/or dieldrin in the past.The
use of dieldrin to control termites was legal in the United States until being banned for this use in
1987. Given the age of your home, treatment using aldrin/dieldrin is a distinct possibility. It is not
clear at this time if the concentration of dieldrin detected around the home's foundation represents
a typical or normal application for termite control.Additional information would be needed to
make that determination.
Because pesticides were detected in the soil samples above the applicable standards, a Health Risk
Evaluation (HRE) was performed by our toxicologist, Dr. Ken Rudo.The HRE cautions against
prolonged dermal (skin) exposure with soils near the house foundation.This would include activities
such as gardening or children playing in the dirt. Some basic precautions you can take to minimize
your exposure include maintaining a good cover of grass, mulching areas where grass does not
grow,washing thoroughly after soil exposure, and removing or wiping shoes to avoid carrying soil
particles into the home.

Dear Mr.Vaughn,
Please find attached additional analytical results for water samples collected from your
water supply well and agricultural hydrant located at the address referenced above, on
September 16, 2013. The sample was submitted for laboratory analyses of nitrate,nitrite,
lead,and arsenic. Nitrate was the only analyte detected in both water supplies as shown on
the attached laboratory sheets.
The following tables summarize the attached analytical results. Detected constituents are
compared with the Federal drinking water standard (Maximum Contaminate Level) and
the NC Groundwater Standard, 15A NCAC 2L.0202.The results are shown in units of
micrograms of contaminant per liter of water and are approximately equivalent to part per
billion (ppb).

Nitrate does not occur above the applicable standard.A Health Risk Evaluation (HRE) of
both water supplies was performed by our toxicologist, Dr. Ken Rudo, for these results

Please find attached additional results for a water sample collected from your well located
at the address referenced above,on October 22, 2013.The sample was submitted for
laboratory analysis of nitrate,lead,and arsenic. Nitrate was the only analyte detected in the
water sample as shown on the attached lab sheets.
The following table summarizes the attached analytical results. Detected constituents are
compared with the Federal drinking water standard (Maximum Contaminate Level) and
the NC Groundwater Standard, 15A NCAC 2L. 0202.The results are shown in units of
micrograms of contaminant per liter of water and are approximately equivalent to part per
billion (ppb).

The permit was renewed in July 2013 and expires 7/3112018.
We could not find the discharge pipe. Mr. Massey pointed out the approximate location of the sub-surface
sand filter. The discharge pipe should be located. According to the file,the original system design did not
include chlorination/dechlori nation. There is no site map available for this system.
It is recommended that the septic tank be pumped every 3-5 years to protect the sand filter.
