Dear Mr. and Mrs. Allen,
Wake County Department of Environmental Services staff, on behalf of the Division of Water Resources,
performed a compliance evaluation inspection of the subject Single -Family Residence Surface Irrigation
System on October 16, 2018. This Notice entails permit condition violations that plus! immediately be
addressed (also refer to the attached inspection report),
The following violations were noted at the time of tale inspection for Condition II. I OPERATION AND
MAIN -I ENANCE REQUIRE TENTS which states, "The facilities shall be properly maintained and
operated at all times."
1. The UV disinfection system was not operating to properly disinfect wastewater effluent. You
indicated that the recent hurricane caused the equipment failure and that a new bulb had been
ordered. Once receive(l, UV Iamp must be replaced and a Spare lamp should be ordered as a
backup if you have not already done so.
2. The sand aster dosing I)LI11117 was not turning oil to apply effluent to the sand filter. Increased
solids and odor was noted from the recirculatiolt/purnp tank as a result. Please repair~ ur replace
the punllp and return the sand filter to normal operation.
1'ou are required to take corrective nleasures regarding the violations listed above items by
November 22, 2018 and send a lettCr to OL11' office Upon completion of the above. Plcase inform our office
if there are extenttating circumstances that preclude compliance with the permit's conditions, or if there
are any ways that we may be of assistance to help you bring this system into compliance. I ailurc to
