On February 6, 2019 Jane Bernard and Gary Kreiser with the Raleigh Regional Office (RRO) of the Division of
Water Resources (DWR), visited the subject facility. The purpose of the Visit was to conduct a follow-up
compliance inspection and determine the status of the repairs required to bring this system back into compliance.
We wish to thank Mr. Crain who was present and assisted during this inspection.
At the time of this inspection the system was not operable, the tanks were full of waste water and in alarm, in
addition both pumps were not working. The facility is working with Scotty's Septic Service to pump and haul
until the repairs are completed.
It is the Divisions understanding that church staff are working with NlacConnell and Associates and that they are
planning on connecting to the neighboring Home Owners Association (HOA) collection system. The draft plans
were e-mailed to the RRO.
REQUESTED RESPONSE:
Please send a letter to our office within 30 days of receipt of this letter. PIease include with your response, your
intent and a timeline for completion of the chosen correction. At the time of this inspection your wastewater
treatment system was not operational, please continue to pump and haul on a regular basis.
Thank you for your attention to this matter. This office requires that the violation, issued October 10, 2017, be
properly resolved in a timely manner. These violations and any future violations are subject to a civil penalty
assessment of up to $25,000.00 per day for each violation. Should you have any questions regarding these

On September 7, 2017, Jane Bernard «ith the Raleigh Regional Office (RRO) of the Dig ision of Water Resources
(DWR), conducted a site visit on the above subject discharge facility. The purpose of the %isit «as to conduct a
compliance inspection. 11'c %%ish to thank Mr. Crain «ho %%as present and assisted during this inspection.
Ut the time of this inspection the breakers (power) %%ere turned off at the control panel. When the breakers were turned on
1e system immediately went into alarm. All tanks were full and the pump tank was seeping_ wastewater to the ground
surface (Figures 1-5). The chlorine tank was at a normal operational lekel and the chlorinator was completel) dry.
Durinti, the inspection. it was obser<ed that the pumps «ere not working in the "automatic'- position. When the pumps
«ere sw itched to the -'hand" position in the panel the wastewater spayed 8-10 foot in the air. Also, a blow off ��al%e was
open (the blow off valve was closed during site %isit), a few fittings were obserNed to be cracked, and wastewater was not
contained within the sand filter. The discharge pipe was inaccessible (FiLllure 6-8).
At the time of this inspection there were no effluent sample results available for the inspector to re%iew.

untreated waste (sewage), bypasses, overfimis and failures to properly disinfect the wastewater must be eliminated.
