RRO staff observed the single family wastewater treatment system was not functioning properly. Dark -colored wastewater and waste solids were observed at the effluent pipe. This waste was discharging into a wetland adjacent to an unnamed tributary to the Upper Little River, Class C waters, in the Cape Fear River Basin. Requested Response: The Division re uests that the permitted wastewater treatment system be re aired or replaced to ensure that the system functions as intended and meets the conditions of vour permit,.including_ meeting the effluent monitorim limits contained in the permit. Please do this within 30 days and submit a written response to this office detailing, how you will ensure in a timely manner that the system is in full comi2liance of the 12ermit conditions and functions as intended. If the system needs significant repairs or replacement. a North Carolina licensed engineer will need to complete an Authorization to Construct (ATC) and submit it to the Division of Water Resources. To apply for an ATC, you are required to submit the following,: a. A letter of request for an ATC to Charles Weaver, NPDES Permitting Unit, Division of Water Quality, 1617 Mail Service Center, Raleigh, NC 27699-1617. b. Plans and specifications of the proposed wastewater treatment system in triplicate. Plans and specifications must be drawn and submitted by a licensed North Carolina professional engineer, If you believe that the system is functioning vroperly. please provide recent effluent monitoring results that e accomplished by contacting a certified lab (provided in the demonstrate that the system is in compliance. This may b packet that was given to you during, the inspection) to perform this analysis and submit the results to this office within 45 days of receipt of this letter. The parameter monitoring limits can be found in Section A of Part I of the general permit and in Attachment B of this letter. Per a brief conversation with Firs. Hart, it was noted that Mr. Hart has passed away. Please complete the attached, "PER -MIT NANiEIONVNERSI[1P CHANGE REQUEST." within 30 days of receipt of this letter. This must be corrected immediately. Please submit a schedule to this office within 30 days of recei t of this letter that states your plan for correcting this deficiency.: A washing machine was hoofed up to an outsides i<_Tot and a discharge pipe was observed to the front yard. EC Correct treatment tablets: if using chlorine tablets for disinfection, you are required to always having chlorine tablets and dechlorination tablets in place. They must be the hind for %%astewater treatment and not for swimming pools. ® Pumping the septic tank: The septic tank should be pumped out every 3 to 5 years. A pumping company can check the status periodically and determine %v hen pumping is required. Receipt of pumping should be submitted to DWR. Receipt of pumping should be submitted to this office. ® Analyzing the effluent: The effluent that is discharged from your system must be analyzed once each year. See Part 1(A) of your permit about his requirement.
