Dear Mr. Salmon:

Division of Water Quality
You are hereby notified that, having been permitted to have a non discharge permit for the subject
animal waste disposal system pursuant to 15A NCAC 2H .0217 and Section 402 of the Clean Water Act,
you have been found to be in violation of your permit.
On April 3, 2003, staff from the Fayetteville Regional Office of the Division of Water Quality
conducted a site visit to the above referenced facility in response to a complaint of waste -runoff from the
sprayfield. Upon arrival at the facility, it was observed that wastewater had runoff during a waste
application event and exited the sprayfield. Waste exited the field in two areas, entering a dry ditch and
ponding in a low area. This is in violation of your permit.
The Division of Water Quality requests that the following item(s) be addressed:
1. Inspect future irrigation events at more frequent intervals to prevent fixture occurrences of runoff.
2. Review Best Management Practices (BMPs) and possibly amend to include flow control structures
to prevent a similar incident in the future.
Failure to comply with the above condition(s) may result in the facility losing its General Permit and
being required to obtain an individual permit for the facility.

On December 21, 1997, staff from the Fayetteville Regional Office of the Division of Water Quality

water occurred.
The Division of Water Quality requests that the following items be addressed:
1. Immediately begin waste application per your certified animal waste management plan (CAWMP) on
