Box 856 Warsaw, NC 28398 SUBJECT: NOTICE OF DEFICIENCY Carroll's Farm 2602-1 Facility No. 09 - 86 Bladen County Dear Mr. Nordin: On June 15, 1999, staff from the Fayetteville Regional Office of the Division of Water Quality conducted a routine inspection of the Carroll's Farm 2602-1 swine facility located in Bladen County. There was quite a bit of trash (bottles, syringes, etc.) observed in the lagoon at the time of the inspection. The General Permit issued to this facility on May 8, 1998 states in condition 11.12 states that "Lagoons/storage ponds shall be kept free of foreign debris including, but not limited to tires, bottles, light bulbs, gloves, syringes or any other solid waste.". A review of the irrigation records indicated that the Plant Available Nitrogen (PAN) rate for the coastal bermuda in crop year 1998 had been exceeded by 63 pounds. It was also noted that the facility failed to have a copy of the Certificate of Coverage (COC) and the General Permit readily available. Permit condition 1.4 states that "A copy of this General Permit, COC, certification farms, lessee and landowner agreements, and the CAWMP shall be readily available at the farm (stored at places such as the farm residence, office, outbuilding, etc.) where animal waste management activities are being conducted for the life of this permit. These documents shall be kept in good condition and records shall be maintained in an orderly fashion.". The Division of Water Quality requests that the following items be performed: 1. Comply with the Certified Animal Waste Management Plan (CAWMP) and the General Permit when applying waste using the correct PAN rates as specified for each crop. Irrigation records should coincide with the CAWMP. 2. Nothing in this letter should be taken as absolving this facility of the responsibility and liability of any violations that have or may result from these deficiencies.
