Please find attached a permit modification request for Site 27 on U-4751 in New
Hanover county. The request is for 12 linear feet of additional permanent stream
impacts to install the 48" RCP and drop inlet at this location. The initial impacts were
calculated incorrectly per the pipe schedule and onsite conditions. This has also beer
submitted through the DWR supplemental electronic process, but I wanted to make
sure that all parties of interest received this request.

Dear Mr. Shaver and Ms. Steenhuis:
As you are aware, the North Carolina Department of Transportation (NCDOT) is currently constructing
project U-4751 (Military Cutoff Extension) in New Hanover County. The purpose of this letter is to request
modifications of the Section 404 Individual Permit and Section 401 Water Quality Certiiication due to an
error in the calculation of the culvert length required at Site 27 (Sta. 205+35 to 211+16 -L- LT & RT). This
modification request includes a revised permit drawing 47 of 83, Wetland Impact Summary sheet 83 of 83
and the DMS Acceptance Letter for 121inear feet of additional permanent stream impacts. Per the original
authorization the mitigation ratio for this site is 1:1.
The initial permitted impacts at this site for the 48" RCP and drop inlet box was reported in the application
to be 5441inear feet. However, the permit drawings and construction plans depicted the pipe and drop inlet
box to be 5481inear feet and the pipe schedule in the construction plans listed the pipe and box length to be
5601inear. Surveyors have staked the site and determined that actual length of permanent impacts required
for the pipe and box at this site is 5561inear feet. The initial 141inear feet of bank stabilization already
authorized for this site will not change.

The purpose of this letter is to notify you that the Division of Mitigation Services (DMS) will provide the addirional
stream mitigation for the subject project. Based on the information supplied by you on February 20, 2019, the additional
stream impacts are located in CU 03030007 of the Cape Fear River Basin in the Southern Outer Coastal Plain (SOCP) Eco-
Region, and are as follows:

These impacts and associated mitigation needs were not projected by the NCDOT in the 2019 impact data. DMS is currently
providing stream mitigation for the impacts associated with this project located in cataloging unit 03030007 of the Cape Feaz
River basin as required by the permits that were issued in June and July 2017, as shown in the below table (in mitigation
credits):

This impact and associated mitigation need were under projected by the NCDOT in the 2019 impact data. DMS will

Instrument dated July 28, 2010.
