Geno,
Here's a summary of the issues discussed during the inspection:
• No monitoring wells installed by the permit schedule of January 1, 2016.
• No backup ORC for facility. William Mcleod, currently listed as BORC, is inactive.
• Soil analysis needs to be done every 3 years. If you had this done, please email a copy to
Michael.

• NDMR form needs to be submitted every month with recorded flow. In March, July and
November, the 3xyear parameters also need to be on the NDMR form (Ammonia, TKN, Nitrate, pH,
Phosphorus).
• A Waste Analysis Report also needs to be submitted March, July and November.
• NDAR form needs to be submitted every month. Put zero if no irrigation (you are currently
submitting this correctly).
• NDMLR form needs to be submitted March, July, and November to report PAN.
I attached all the forms and a copy of the permit on a 1-2-19 email, but I will forward that email to you
again.
