Inspection Summary:
Michael Mellinger and Helen Perez of DWR inspected AJ washpit with Geno Kennedy (ORC) on 03/22/2019. The following
non-compliance issues were observed.
1. Referring to Section 1.2 Monitoring wells that were required in the permit to be placed on the facility by January 1, 2016
were not installed.
2. Referring to section 3.11 public access to the irrigation sites and wastewater treatment facilities shall be prohibited, there
were no restrictive signs or barriers.
3. Referring to section 3.1 the facility shall be properly maintained and operated at all times, poor condition of wash pad,
pump area, and lagoon.
4. Referring to section 3.2 failure to have a designated BORC.
A NOV will be issued for Permit Number WQ0012630 for the above non-compliance issues.

Comment: No wells have been placed on the property, permittee feels that wells are not necessary. There
are no restriction sians or barricades around the facilitv. The storaae treatment laaoon was
located closer to the property line than that was allowed by adminstrative code that was
applicable at the time of permitting. The site is in poor condition, cement pad was broken on the
back side next to the lagoon and cracks in the concrete on the floor. Trash in the lagoon and pipe
leadina from wash Dad to the laaoon. exposed wires at the DumD. and hoses lavina around the
ump and wash pad that are deteriorated
