As ------------- E �D �E a if 1 SURF m B-OROCK \NEL� NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT APPLICATION FOR PERMIT TO CONSTRUCT MONITOR/RECOVERY WELL(S) To: NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION January 23 1986 Gentlemen: BEDROCK MONITORING WELLS - 13 WELLS In accordance with the provisions of Article 7, Chapter 87, General Statutues of North Carolina, and regulations pursuant thereto, application is hereby made by Chemtronics Inc. for a permit to construct a name of well owner monitor/recovery well(s) as described below and in the accompanying data submitted as a part of this application. (a) Name of property owner: Chemtronics, Inc. (b) Location of property: 180 Old Bee Tree Rd. Swannanoa Buncombe N.C. Identify roads with State Highway" road identification numbers. (Show all existing water supply wells within a radius of 1,000.feet of the proposed well.) All .grouting materials around the well casings must be emplaced by either the pumping or pressure method according to requirements of Section .0107(d) (2) (A) (B) of the Criteria On Standards Applicable To Water Supply And Certain Other Type Wells (15 NCAC 2C) . 3. The eight-inch outer steel casing shall meet or exceed the specifica- tions of ASTM A-53, A-120, or A-589 and have a minimum wall thickness of 0.250 inches. 4. Well completion, well abandonment, and water quality reports are to be submitted to the Division of Environmental Management's Groundwater Section Central Office in Raleigh. 5. All wells shall be afforded reasonable protection against damage during construction and use. 6. When the well has served its purpose and is of no further use, it shall be permanently abandoned according to the requirements of Section .0113 of the Criteria On. FOR THE CONSTRUCTION OF A MONITOR WELL SYSTEM located in the Bedrock System of the Blue Ridge-Inner Piedmont Hydrogeologic Unit on the property of Chemtronics, Inc. ,. off Old Bee Tree Road in Buncombe County, in accordance with the application dated January 23, 1986, and in conformity with specifications and supporting data, all of which are filed with the Department of Natural Resources and Community Develop- ment and are considered a part of this Permit. This Permit is for well construction only, and does not waive any provisions or requirements of any other applicable laws or regulations. Construction of a well under this Permit shall be in compliance with the North Carolina Well Construction Regulations and Standards, and any other State and local laws and regulations pertaining to well construction. This Permit will be effective from the date of its issuance until July 23, 1986, and shall be subject to other specified conditions, limitations, or exceptions as follows: 1. If any parts, requirements, or limitations contained in this Permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within 30 days following receipt of this Permit, identifying the specific issues to be contended. Unless such demand is made, this Permit shall be final and binding.
