with the application elated August. 21, 1980 and in annformity with specifications and
supporting data, all of which are filed with the Department of fiatural resources .and
Cor,,uni tyr level op nt and are considered a part of this Permit.
This Permit is for well construction only., and does not waive any provisions or
requirepants of the Water Use Act of 1967,L,or any other applicable lams or regulations.
Cc4istruction of a well under this Permit shall be -in compliance with the North
Carolina Well Construction Regulations and Standards, and any other laws and regulations
pertaining to well construction.
This Perr+it will be effective from the date of its issuance until February 21,
1981 and shall be subject to other specified conditions, limitations, or exceptions
as follows:.
1. Prier notice of the Pumping test .date gust be given to Roy H. Davis, P. 0.
Sox 370, Ashovilley N. C. 253-3341 at least 46 hours prior to the start of
the test.,
2. Well site approval must be obtained from the Division of Health Services of
the N. C. Department of. Human Resources in Black mountain, N. C.
