In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and other
applicable Laws, Rules and Regulations,
PERMISSION IS HEREBY GRANTED TO

FOR THE CONSTRUCTION OF One (1) monitoring well (MW-13) in the Blue Ridge Geologic Belt, located on
property owned by Mr. Robert D. Clark (PIN 183614449565) in accordance with the application dated February 8,
2010, and in conformity with specifications and supporting data, all of which are filed with the Department of
Environment and Natural Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or requirements of any other
applicable laws or regulations.
Construction of a well under this Permit shall be in compliance with the North Carolina Well Construction
Regulations and Standards, and any other State and local laws and regulations pertaining to well construction.
This Permit will be effective for the construction of a monitoring well from the date of its issuance and shall be
subject to other specified conditions, limitations, or exceptions as follows:
1. You are required to provide a copy of this permit to the property owner;
2. You are required to provide notification to the property owner upon well completion and upon termination of
monitoring;
3. You are to provide 24 hours notice to the APS ARO prior to well construction. Notice may be provided via email
or by calling 828.296.4684;
4. The monitoring well shall be maintained in accordance with 15A NCAC 2C .0108; and,
5. The well shall not hydraulically connect separate aquifers.

27, 2010 issued for the construction of a monitoring well as described in the application and attached
