Natural Resources and are considered a part of this Permit.
This Permit is for well construction only,and does not waive any provisions or requirements of any other
applicable laws or regulations.
Construction of wells under this Permit shall be in compliance with the North Carolina Well Construction.
Regulations and Standards, and any other State and local laws and regulations pertaining to well construction.

Construction Permit No. WM 0100207 dated June 10, 2009, issued to Springer Eubank Company for
Scotchman #125, for the construction of one (1) monitoring well. This well will be drilled on adjacent
property owned by Wachovia Bank on 350 Cranberry Street, Newland,Avery County, NC.
This Permit will be effective from the date of its issuance and shall be subject to the conditions and
limitations as specified therein. Attached is a template Well Construction Record (GW-1a) to be completed
