Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: The proposed roadway improvements occur as much as practicable within the footprint of the existing roadway and the esting NCDOT right-of-way. Stream impacts have been minimized as much as practicable and are necessary to improve drainage on the project. Wetland impacts have been minimized as much as practicable and hand clearing is proposed wherever practicable. 1b. Specificallydescribe measures taken to avoid or minimize the proposed impacts through construction techniques: 3i. Total jurisdictional ditch impact in square feet: 4g. Total temporary open water Impacts: 4g. Total permanent open water impacts: 0.00 0.09 All in-stream v.nrk will be done "in the dry" or using a temporary coffer dam. All necessary sediment and erosion control BMPs will be installed and maintained through construction. Disturbed ground will be restored and stabilized with permanent vegetative cover. NCDOT Best Management Practices will be implemented during all phases of construction. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? C' Yes r No 2b. If this project DOES NOT require Compensatory Mitigation, explain why: The proposed project will result in 63 linear feet of permanent impact to perennial stream channel and 0.04 acre of permanent impact to wetlands resulting from fill, excavation, or mechanized clearing. Mitigation is not proposed for impacts to jurisdictional ditches or hand clearing in wetlands. F. Stormwater Management and Diffuse Flow Plan (required by DWR) " I 1. Diffuse Flow Plan 1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules? C' Yes r No If no, explain why: Project is located in the White Oak River Basin. 2. Stormwater Management Plan 2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?* r Yes ' No Comments: G. Supplementary Information 1. Environmental Documentation 1a. Violations (DWR Requirement) 2a. Is the site in violation of DWR Water Quality Certification Rules (15A NCAC 2H .0500), Isolated Wetland Rules (15ANCAC 2H .1300), or DWR Surface Water or Wetland Standards or Riparian Buffer Rules (15A NCAC 2B .0200)? * f Yes r No 3. Cumulative Impacts (DWR Requirement) 3a. Will this project result in additional development, which could impact nearby downstream water quality?* Yes r No 3b. If you answered "no," provide a short narrative description. The proposed project will not result in downstream impacts or growth-stimulating effects. The ewsting uses of streams within the project area (C;NSW) should not change as a result of this project. 4. Sewage Disposal (DWR Requirement) 4a. Flood Zone Designation (Corps Requirement) Sa. Will this project occur in a FEMA-designated 100-year floodplain?* Yes r No 2.81 MB 1.47 MB Sb. If yes, explain how project meets FEMA requirements: The proposed project will require work within a regulatory floodway and 100-year floodplain. However, the impacts are not considered substantial, now would they affect the elevations of the streams that cross the project study area. Therefore, neither a MOA or CLOMR are anticipated for this project. 8c. What source(s) did you use to make the floodplain determination?* wv.ncfloodmaps.com Miscellaneous Comments NCDOT will apply for a CAMA Major Permit for impacts to AEC areas. These areas are listed in the impact summary table under "notes" and the AEC areas are included in the permit drawings. Miscellaneous attachments not previouslyrequested.
