Notice of Violation and Intent.to Assess Civil Penalty
Dear Ms. Kiang:
Aqua North. Carolina, Inc. (Aqua) offers the follo wing response to the above
referenced Notice of Violation and Intent to Assess Civil Penalty. This was for
violations that occurred* in December 2018:
AQUA mistakenly submitted the December eDMR with: lab results that did. not agree
with the report fi-om the lib.. in both cases this caused an inadvertent exceedance
that was not caught until we reviewed the hard copy of the eDMR. This has been
amended and corrected on the eDMR and will: be -submitted correctly on the hard
copy that is mailed to the state. We apoliog'e z: for the additional paperwork- that this
has caused. The lab- results -reports have been emailed to Dean Hunkele of the local
Wilmington office aloe with a- similar explanation on: February 1, 2019.
'there was an issue with 2 of the 'ASS results entered into the EDMR vs the lab
results, These results were entered manually vs our typical process of importin

the ED-D File. The lab EDD file at times has some lag being received. The December
DMR was compliant,
The TSS results Were 4.1 not 401 and 6.2 not 62 on the 4th and 121hrespectilvely.
