referenced. Notice of Violation and Intent to Assess :Civil Pena.11ty. This was for
violations that occurred in December 2018:

AQUA mistakenly submitted the December eDMR with a lab results that did not
agree with the report from the Lab., This caused an][nadvertent exceedance that was
not caught until we reviewed the hard copy of the eDMR, This has been amended
and corrected on the eDMR and w"Il be submitted correcd on the hard copy that is

mailed to the state. We apologize for the additional paperwork that this. has caused,
The lab results reports have been emailed to Dean Hunkele of the local Wilmington
office along with a similar explanation on February 1, 20190
There was an issue with .1 of the Nitrogen, Ammonia Total entered into, the EDMR vs
the lab results, These results were entered. manu I Y Vs ou a, I r v t'vi ca I process of

The N.result was 4 not 40 on the 121h.
AQUA is looking into issues .and solutions for the Enteroco i exceedance on. the 18th
of December. The UV bank has a sump on the 9.0 degree turns between the 2 sets of
bulbs.. These areas seemed to cause some of the issue in collecting fine particles
and organic. growth. AQUA has increased cleaning on these areas of the U'V bank.

The filters are chemically treated weekly on Mondays. The filters are replaced as
needed. Beau Rivage hasn't experience an exceedance since the operational
improvement to date.
