The MRO would like to thank you and Mr. Hadley for your time during the site visits. The treatment and irrigation systems appeared to be operating adequately at the time of inspection. Please note the following comments to maintain overall program compliance: • The audible and visual alarms for the Ultra Violet (UV) unit need a sensor installed, because this unit is NOT connected by the auto -dialer. The inoperable alarms do NOT impact UV treatment, which appears operational, with bulbs replaced on 6-5-18. • The permit is currently in the renewal process with our Central Office staff in Raleigh. Renewal questions may be directed to Sonia. Graves(o)_ncdenr.gov. There were only occasional missed weekly checks. Mr. Spencer also accessed the UV unit and showed MRO staff the field site, which appeared well maintained for underbrush on this wooded application site. Mr. Spencer confirmed he was okay with MRO contacting their former engineering company, should additional information be necessary for this permit renewal currently in process with Sonia. rg aveskncdenr.gov. The owner contracts with AQWA for system service. MRO staff met the AQWA technician (Jonathan Hadley) on 8-27- 18 for a complete system review. All treatment and irrigation components appeared operational at the time of inspection. A new dial type flowmeter for the irrigation field was installed. Dripline repairs were completed in all three zones. The UV audible and visual alarms were not functional and need a sensor installed, as these alarms are not connected to the auto -dialer (as noted in previous inspection reports). The AQWA tech did not have the appropriate sensor on hand for immediate repair. The UV system appears operable, with recent bulb replacement on 6-5-18. MRO staff attempted to gather updated coordinate data, but equipment readings gathered under tree canopy were inadequate when checked with mapping applications. MRO staff spoke with David DeCaron, P.E. (Piedmont Design) concerning the need for an as -built map, on 8-29-18 and will have Central Office staff include him on the additional information request. Please note the following comments to maintain program compliance.: • Items noted in the cover -letter. • The MRO should be informed of any changes to the contact information noted above. Comments: Septic tank residuals were at 18inches, pumper schedule is assessed by Tammy Sanders (AQWA) based on tank dimensions. Disinfection Tablets Present in chlorinator? Proper Size? Y UV System N Spare bulb on hand? Comments: Contractor replaces UV bulbs annually. A new bulb was installed on 6-5-18. The UV system is NOT connected by telemetry and a sensor is needed for the Audible and Visual alarms to function. The owners will need to contact AQWA for repair should they find the UV inoperable during their weekly inspection. Do not view the UV bulb for inspection. It is unsafe for your eves. Per conversation with your contractors, you can feel the UV controller box for heat to determine if the unit is functional. Call your contractors forfurther assistance. N Are restrictions for use of these areas specified? This is a residential property. The standard two wire fence is in place. Y Is the permit being followed? It appears to be. The permittee logs inspections by computer. Only occasional weekly inspections are missed. Y Is site access restricted in accordance with the permit? If so, how? With fencing, of which needed routine repair for deer traffic. Y Is the permit available upon request? NE Are operational logs present? The permittee logs were not viewed and are requested to be emailed. NE Are operation and maintenance records present? The contractor had their records, but owner records were not examined. N Has the permittee received any public complaints in the last 12 months? Comments: Owner was aware of a downed tree and has plans to remove.
