All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. All MDL (LOD) and RDL (LOCO values reported for environmental samples have been Yc
corrected for the dilution factor used in the analysis. All radiochemical sample results for solids are
reported on a dry weight basis with the exception of tritium, carbon-14 and radon, unless wet weight
was requested by the client. Ali Method and Batch Quality Control are within established criteria 55
except where addressed in this case narrative, a non-conformance form or properly qualified within the
sample results. By my digital signature below, I affirm to the best of my knowledge, all
problems/anomalies observed by the laboratory as having the potential to affect the quality of the data
have been identified by the laboratory, and no information or data have been knowingly withheld that
would affect the quality of the data.

The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not

Analyte
The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.

If the sample matrix contains an interfering material, or if concentrations of analytes in the sample are higher than the

highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value
different than 1 is used in thisfield, the result reported has already been corrected for this factor.

These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal

Original Sample

Quality Control
This section of the report includes the results of the laboratory quality control analyses required by procedure or

analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material,

The purpose of this letter is to satisfy the 5-day reporting requirement in accordance with Part I11, Section
9 of the referenced permit.
On February 12, 2016 at approximately 10:15 am, facility personnel observed that the water level in a
stormwater retention pond had dropped to the bottom of the outlet valve, which indicated the valve had
been opened. Water in the pond was observed to be at a higher level above the valve earlier in the week,
but water contained in the pond was not discharged to the creek due a light emulsion on the surface. It is
unknown when or who opened the valve. The quantity of the discharge is unknown, but estimated at
4,000 gallons. The source of the emulsion on the pond was discovered to be a tote of latex that was
accidently punctured and leaked into a stormwater catch basin.
The valve was closed immediately, and facility personnel contacted Haz-Mat Environmental Services of
Charlotte, NC to mobilize crews and vacuum trucks to abate the spill. Approximately 18,575 gallons of
water was pumped from the retention pond, and transported to the Haz-Mat Environmental Services
centralized waste treatment facility in Charlotte, NC for disposal. Non -Hazardous Special Waste
Manifests are attached. Facility personnel observed a milky discoloration in the receiving creek
(Kennedy Mill Creek) under W. English Road and Westchester Drive, and west of Hodgin Street.
Total solids, pH, and residual polymers were measured in a sample from the retention pond. The pH was
reported as 7.8 standard pH units. Total solids and residual polymers were not detected in the wastewater
sample. The laboratory report is attached.
Piedmont Geologic, on behalf of Ennis -Flint contacted the NCDEQ by telephone on February 12, 2016 at
1:15 pm to report the discharge. When unable to reach the NCDEQ, Piedmont Geologic contacted NC
Emergency Management to report the spill. Relevant site information was collected by Mike Bennett at

Analysis of Pond Water

Method: A quart sample of pond water was collected on 2/12/16, A 2 mL solution was prepared by
diluting with methyl ethyl ketone (MEK) in a 10:1 ratio. A blank of the same MEK was run, and was then
followed by the diluted pond water sample. The results of the blank and the pond water sample were
compared on the basis of methyl methacrylate (MMA) and butyl acrylate (BA) concentrations. The
samples were run on a Thermo Scientific Trace 1310 Gas Chromatographer using a Zebron ZB-5 Capillary
column.
The pH and percent of non-volatile mass (NVM) of the pond sample were also tested. The pH was
tested using an Accumet 110 pH meter. The percent of NVM was tested using a Sartorius Moisture
Analyzer Mark 3 at a temperature of 150 °C.

As indicated by the table, the pond water sample contributed no additional response to either MMA or
BA. Therefore, we conclude that there must have been no MMA or BA in the pond water, or, at least, in
low enough concentrations as to be indiscernible by our detector.

Conclusion: Our machine has found no detectable traces of MMA or BA in the pond water. Also, we have

All sample aliquots were received at the correct temperature, in the proper containers, with the
FP
appropriate preservatives, and within method specified holding times. All MDL (LOD) and RDL (LOO)
values reported for environmental samples have been corrected for the dilution factor used in the FC1
analysis. All Method and Batch Quality Control are within established criteria except where addressed
in this case narrative, a non-conformance form or properly qualified within the sample results. By my digital signature below, I affirm to the best of my knowledge, all problems/anomalies observed by the rss]
laboratory as having the potential to affect the quality of the data have been identified by the
laboratory, and no information or data have been knowingly withheld that would affect the quality of the
data.
