THE CONTRACTOR SHALL SCHEDULE A PRE -CONSTRUCTION CONFERENCE WITH JOHNSTON COUNTY AT LEAST 2 WEEKS PRIOR TO STARTING CONSTRUCTION BY CONTACTING MIKE KEEN AT (919) 989-5075 2. CONSTRUCTION OF THIS PROJECT SHALL BE PERFORMED IN ACCORDANCE WITH THE CURRENT EDITION OF THE JOHNSTON COUNTY PUBLIC UTILITIES DESIGN AND CONSTRUCTION TECHNICAL STANDARDS AND WITH THE CONTRACT PLANS AND SPECIFICATIONS. ALL WORK SHALL BE PERFORMED IN ACCORDANCE WITH THE NORTH CAROLINA DEPARTMENT OF LABOR OCCUPATIONAL SAFETY LAWS. 3. UNDERGROUND UTILITIES ARE APPROXIMATE IN LOCATION. IT IS POSSIBLE THAT ALL EXISTING UTILITIES HAVE NOT BEEN SHOWN ON DRAWINGS. CONTRACTOR SHALL VERIFY SIZE, DEPTH, LOCATION, AND MATERIAL OF ALL EXISTING UTILITIES PRIOR TO THE START OF CONSTRUCTION. ALL STAKING, RESTAKING, AND CUT SHEETS SHALL BE PERFORMED BY A NC REGISTERED PROFESSIONAL LAND SURVEYOR. 6. ALL PIPE ELEVATIONS ARE TO INVERT OF PIPE UNLESS OTHERWISE SPECIFIED. 7. NO EXCAVATED MATERIALS ARE TO BE PLACED IN WETLANDS, STREAMS, OR STREAM BUFFERS. 8. DO NOT EXCAVATE/TRENCH BEYOND WHAT CAN BE REPAIRED AT THE END OF EACH WORK DAY. 9. NO DISTURBANCE IS PERMITTED BEYOND THE LIMITS OF DISTURBANCE. 10. ALL AREAS DISTURBED BY THE CONTRACTOR'S CONSTRUCTION SHALL BE RESTORED TO A CONDITION AT LEAST EQUAL TO THAT WHICH EXISTED PRIOR TO CONSTRUCTION. 11. THE CONTRACTOR SHALL VIDEOTAPE THE PROJECT AREA AS DETAILED IN THE SPECIFICATIONS FOR THE PURPOSE OF DOCUMENTING PRE -CONSTRUCTION CONDITIONS. THE VIDEOTAPE SHALL BE PROVIDED TO JOHNSTON COUNTY PUBLIC UTILITIES. THE VIDEOTAPE SHALL BE IN DVD FORMAT. 12. ALL MANHOLES WILL HAVE A 5 -FOOT INSIDE DIAMETER AND BE EQUIPPED WITH STANDARD JOHNSTON COUNTY WATER TIGHT FRAME AND COVER AND INTERNALLY COATED WITH A COUNTY APPROVED PRODUCT (REFERENCE PROJECT SPECIFICATION MANUAL FOR APPROVED PRODUCTS AND APPLICATION TYPES.) EROSION CONTROL NOTES 1. NO CONSTRUCTION ACTIVITIES SHALL COMMENCE PRIOR TO OBTAINING AN EROSION AND SEDIMENT CONTROL PERMIT. 2. CLEAR SITE ONLY AS NECESSARY TO INSTALL EROSION CONTROL DEVICES. INSTALL EROSION CONTROL DEVICES AS SHOWN ON THE PLAN AND IN ACCORDANCE WITH THE CONSTRUCTION SEQUENCE ON SHEET 12 - DETAIL SHEET. 3. INSTALL TREE PROTECTION FENCE ALONG THE PERIMETER OF THE DISTURBED AREA WITHIN RIPARIAN BUFFER AND WETLAND AREAS AS SHOWN ON THE PLANS. 4. NOTIFY ENGINEER AND NC DEMLR (919-791-4200) AT LEAST 48 HOURS PRIOR TO COMMENCING GROUND DISTURBING ACTIVITIES. ONCE EROSION CONTROL DEVICES ARE APPROVED, CONSTRUCTION ACTIVITIES MAY COMMENCE. 5. CONTRACTOR SHALL IMPLEMENT A SELF INSPECTION PROGRAM TO SATISFY THE REQUIREMENTS OF THE SEDIMENTATION POLLUTION CONTROL ACT AS AMENDED IN 2006. INSPECTION OF SEDIMENT AND EROSION CONTROL DEVICES SHALL BE INSPECTED AFTER EACH PHASE OF CONSTRUCTION IS COMPLETE AND CONTINUED UNTIL PERMANENT GROUND COVER IS ESTABLISHED IN ACCORDANCE WITH NCGS 113A-54.1 AND 15A NCAC 4B.0131. THE SELF -INSPECTION FORM IS AVAILABLE AS MSEXCEL SPREADSHEET FROM HTTP://DEQ.NC.GOV/ABOUT/DIVISIONS/ENERGY-MINERAL-LAN D-RESOURCES/EROSION-SEDIM ENT-CONTROL/FORMS. 6. A COPY OF THE APPROVED PLAN, THE CERTIFICATE OF APPROVAL, LETTER OF APPROVAL, AND INSPECTION FORMS ARE TO BE KEPT ON SITE AND AVAILABLE AT ALL TIMES. 7. DISTURBED AREAS SHALL BE STABILIZED WITHIN THE STABILIZATION TIME FRAMES AS SHOWN ON THE DETAIL SHEET. 8. ANY OPEN CUT STREAM WORK SHALL BE PERFORMED IN THE DRY. OPEN CUTS SHALL UTILIZE STREAM PUMP AROUND AND DEWATERING BAGS AS SHOWN ON THE DETAIL SHEET. 9. ANY SEEPAGE PUMPED OUT OF BORE PITS OR TRENCHES SHALL BE PUMPED THROUGH A DEWATERING BAG. 10. LIMIT DISTURBANCES TO THE LENGTH THAT CAN BE STABILIZED AT THE END OF EACH WORK DAY. 11. AFTER SITE IS PERMANENTLY STABILIZED, REQUEST AN INSPECTION BY THE ENGINEER AND NC DEMLR.
