On June 4, 2018, Sue White, Alaina Morman, and Brandon Wise, inspectors with the North
Carolina Department of Environmental Quality — Division of Energy, Minerals and Land
Resources (DEMLR) met with Greg Brown and Kevin Gwyn at the facility located at 1510
Carter Street in Mt. Airy, to conduct a compliance inspection for the facility's Industrial
Stormwater Permit. This inspection consisted of reviewing the: Stormwater Pollution Prevention
Plan (SPPP) and each of its components, Monitoring Records, facilities outfalls, containment
facilities as well as review of the overall condition of the site.
Permit:
This facility holds Individual Stormwater Permit NCS 000346 to discharge stormwater from
industrial activity under the National Pollutant Discharge Elimination System (NPDES). A copy
of the current permit is required to be maintained with the Stormwater Pollution Prevention Plan
(SPPP) documents. The current permit is under review. The site is operating under the previous
permit until the current one is issued.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. All monitoring records are required
to be maintained with the SPPP for a minimum of five (5) years. Inspectors reviewed the SPPP
and each of its components. Documents appeared to be mostly complete, however, there was a
lack of records for any inspections that occurred during 2017.

Facility Site Review:
Mr. Brown and Mr. Gwyn gave the inspectors a tour of the facility operations and to view all
outfalls. Insteel at this location builds steel reinforcement structures for use throughout industry.
Their clients include government entities as well as large private sector businesses. They
construct the steel skeleton in walls, floors, bridges, etc. Upon inspection of the grounds it was
determined that all outfalls had not been identified. Additionally, this facility was granted
Representative Outfall Status (ROS) before the proper two sampling events were conducted for
Outfalls 003 & 004.
Effluent Receiving Waters:
Stormwater from this facility drains to Lovills Creek, Class "C" waters of the Yadkin River
Basin. It is suspected that one of the outfalls may drain directly to the Ararat River. The outfall
labeled as Outfall-003 drains to an unknown location as determined during the site inspection,
therefore it is unknown what the receiving waters are for that outfall.
Self -Monitoring Pry
This facility is required to perform both qualitative and analytical monitoring twice annually per
permit requirements. With the exception of 2017, Insteel Wire Products has been monitoring as
required and documents that were provided appeared to be mostly complete and up to date.
There have been issues with elevated results for aluminum and zinc, however, a previous study
determined the source to be off -site. Some issues were also observed with elevated total
suspended solids results.
The following determination and recommendations were made by DEMLR Staff.
• This facility was granted representative outfall status for Outfall-002 starting in 2009.
Results of this inspection require that the ROS be rescinded until a site evaluation
identifying any additional outfalls are conducted and a full year of qualitative and
analytical monitoring of all outfalls has been completed. At that time the data will be
reviewed if representative outfall status is requested.
• Perform a dye test on Outfall-003 in order to locate where the outfall discharges. The
outfall should be re-evaluated following the locating of the discharge point.
• Provide a spill kit to be located with the oil -water separator for emergency response.
• Ensure that all outfalls have a proper clearing where a sample can be collected.

Comment: All listed outfalls were inspected, however new outfalls were observed on the north side of the
property. The facility had Representative Outfall Status during the inspection, however, following
the inspection, the decision was made to rescind that status.

On August 23, 2012, Gary Hudson of this office met with you to perform an Individual
Stormwater Permit Inspection at the Insteel Wire Products facility located on Carter Street in
Mount Airy. This inspection evaluated the areas as designated on the attached inspection form.
Observations from each area are addressed, below:
1. Permit
Insteel Wire Products holds an NPDES permit # NCS000346 for the discharge of stormwater
from the facility. The General Permit became effective ,tune 12, 2009 and will expire June 30,
2014. Please be advised that you will continue to operate under the conditions set forth in this
permit, until the new permit is issued to you.
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). A copy of the SPPP was available for review. The plan contained all of the items
required by the stormwater permit. Please note that the permit requires the plan to be reviewed
and updated on an annual basis.

Insteel Wire Products was recently granted Representative Outfall Status for outfall number
002. Observations of the outfali at the facility did not reveal any concerns. The site appeared
very clean and well managed.

Dee River Basin.
5. Monitoring Program
Part II, Section C of the permit requires that semi-annual qualitative (visual) monitoring be
performed and documented once in the spring (April - June) and once in the fall (September —
November). A review of the facility files found that the qualitative monitoring has been
conducted as required.
Part li, Section B of the permit requires that analytical monitoring be performed. A review of the
facility files found that the analytical monitoring has been conducted as required.
The analytical monitoring for on -site vehicle maintenance listed in Section B of the permit is not
required since the Insteel Wire Products plant uses less than 55 gallons of new motor oil per
month when averaged over the calendar year.

On Match 02, 2016 Glen White of the North Carolina Department of Environmental Quality —
Minerals & Land Resources met with you at the subject facility located at 1510 Carter Street in
Mounty Airy to conduct a compliance inspection for the facilities Industrial Stormwater Permit.
Your cooperation and assistance with the inspection were greatly appreciated. Findings are
addressed below:
Permit:
This facility holds Individual Stormwater Permit NCS 000346 to discharge stormwater from
industrial activity under the National'Pollutant Discharge Elimination System (NPDES). A copy
of the current permit is required to be maintained with the Stormwater Pollution Prevention Plan
(SPPP) documents. The current permit was available for review at the time of this inspection.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. Inspectors reviewed the SPPP and
each of its components. Documents appeared to be complete and up to date. There have been
issues with elevated results for Aluminum and Zinc. A study was done to find the source. Study
found that materials coming into the facility are the likely cause. Representative Outfall Status
was previously granted and allows Outfall 002 to be RO for 001, 003 and 004.
Facility Site Review:
Mr. Mills gave the inspector a tour of the facility operations and to view all outfalls. Insteel at
this location builds steel reinforcement structures for use throughout industry. Their clients
include government entities as well as large private sector businesses. They construct the steel
skeleton in walls, floors, bridges, etc. Some applications are used in bomb shelters, prison walls
and government buildings and bridges.

Effluent Receiving Waters:
Stormwater from this facility drains to Lovills Creek, Class "C" waters of the Yadkin River
Basin.
Self -Monitoring Program:
This facility is required to perform both qualitative and analytical monitoring twice annually per
permit requirements. Insteel Wire Products has been monitoring as required and documents
appear to be complete and up to date. There have been issues with elevated results for
Aluminum and Zinc. A study was done to find the source. They Study found that materials
coming into the facility are the likely cause. Hay bales have been used at the outfall location to
filter out contaminants and solids.
The facility was found to be compliant with its industrial stormwater permit requirements.
Please continue to monitor as initially instructed unless further monitoring is requested due to the
