Inspection Summary:
A routine compliance inspection of Majestic Oaks WWTF was conducted on 03-06-2019.The purpose of the evaluation was
to verify that the facility is operating in compliance with the conditions and limitations specified in Permit WQ0030088. Don
Feller, ORC,was present during the inspection. Don is a contract operator from Envirolink and has been ORC since the
summer of 2018.
Currently,the facility is treating an average of 30,000gpd of the 59,720gpd permitted flow. Effluent disposal consists of two
high-rate infiltration basins, Basin#1 located on site of the treatment plant and Basin#2, located down the street in the
neighborhood of Majestic Oaks. Operator and maintenance logs were well organized and detailed.The effluent flow meter
was calibrated on 5-10-18 and the turbimeter and infiltration basin flow meters were calibrated on 4-20-18.
Since the end of 2017,the facility has had numerous monitoring violations, especially with Total Nitrogen and Total
Suspended Solids. I recommended to the ORC to call Tom Tharrington,the Wilmington Regional Office,Wastewater
Consultant for technical assistance. Don called and had a technical assistance visit from Tom 2 days after this inspection
and another visit is scheduled.
The following non-compliance issues were observed during the compliance evaluation.A Notice of Violation/Notice of Intent
will be issued:
1. Freeboard in Basin#2 was less than 2 feet.The waste level was at the very top of the berm wall in some locations and
erosion of the inside of the embankment was observed.According to permit section 111.9,freeboard shall not be less than two
feet at any time.
2.Well established trees and shrubs were growing on the inside embankment of Basin#2. Lack of maintenance of this
basin has been mentioned in the 2015 and 2017 inspection reports. According to permit section 111.11,trees, shrubs and
other woody vegetation shall not be allowed to grow on the earthen dikes or embankments; and section 111.1,the facilities
shall be properly maintained and operated at all times.
3.There were no freeboard gauges in the infiltration basins or the 5-day upset pond.According to permit section 111.10,
gauges to monitor waste levels in the infiltration basins and 5-day upset pond shall be provided.
4.The floor grating around the splitter box/bar screen area was in an advance state of corrosion causing a safety hazard.
According to permit section 111.1,the facilities shall be properly maintained and operated at all times.

Comment: Generator was automatically activated when power was interrupted but stopped operating within
a minute. Contractor was called and arrived to investigate and fix the issue durinq the

Comment: The UV system is on and effluent passes through but there is zero intensity. The system has not
been used for disinfection at least since the current ORC started last summer. The ORC uses
the two backup tablet chlorinators for disinfection.

Comment: Operator and maintenance log was detailed and complete. Monitoring reports and lab data
reports can be accessed on ORC's computer. Since 2017, there have been six violations where
effluent concentrations exceeded GW standards and two violations in the goundwater lowering
system effluent for infiltration basin#1.

Comment: The gauge to monitor waste levels in the 5-day upset pond was knocked down during Hurricane
Florence and has not been replaced. There are no required gauges in the two infiltration basins.
There were many trees and shrubs growing on the earthen embankment of basin #2, located off
site in the neighborhood. The waste level of Basin #2 was at the top of the embankment in some
areas. Erosion of the inside wall of the embankment was observed that further weakens the
berm. These issues in Basin #2 are a big concern, considering the location and the berm wall is
above grade by—5-6 feet. There was no knowledge of removal of any deposited materials that
could impede the infiltration process for Basin#2.

Comment: There was excessive grease built up in the corners of the EQ basin, including the corner where
the high level floats were located. The high level visual and audio alarms were operational when
float was pulled during the inspection, however, the grease should be removed as to not become
an issue with the operation of the floats.

The North Carolina Division of Water Resources conducted an inspection of the Majestic Oaks WWTP on March
6, 2019. This inspection was conducted to verify that the facility is operating in compliance with the conditions
and limitations specified in Non-discharge Permit No. WQ0030088. A summary of the findings and comments
noted during the inspection are provided in the enclosed copy of the inspection report.
The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Wilmington
Regional Office. The following violation(s) were noted during the inspection:

A Notice of Violation/Intent to Issue Civil Penalty is being issued for the noted violation of North Carolina General
Statute (G.S.) 143-215.1 and the facility's Non-discharge Permit. Pursuant to G.S. 143-215.6A, a civil penalty of
not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or
fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S.
143-215.1.
If you wish to provide additional information regarding the noted violation, request technical assistance, or discuss
overall compliance please respond in writing within ten (10) business day after receipt of this Notice. A review of
your response will be considered along with any additional information provided. You will then be notified of any
civil penalties that may be assessed regarding the violations. If no response is received in this Office
within the 10-day period, a civil penalty assessment may be prepared.
Remedial actions should have already been taken to correct this problem and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law. If the violations are of a continuing nature, not related to operation and/or maintenance problems, and
you anticipate remedial construction activities, then you may wish to consider applying for a Special Order by
Consent.
