GENERAL AND SITE VISIT INFORMATION l. Was a site visit conducted?® Yes or❑No a. Date of site visit: 3/14/19 b. Site visit conducted by: Rick Trone,Ashley Kabat C. Inspection report attached? ® Yes or® No d. Person contacted: Yvonne Couts and their contact information: 9( 19)545 -5848 ext. e. Driving directions: 955 Corinth Road,Moncure,NC 2. Discharge Point(s):NA Latitude: Longitude: Latitude: Longitude: 3. Receiving stream or affected surface waters: NA Classification: River Basin and Sub-basin No. Describe receiving stream features and pertinent downstream uses: U. EXISTING FACILITIES: l. Are there appropriately certified Operators in Charge(ORCs) for the facility? ® Yes❑ No❑N/A ORC: Yvonne Couts Certificate#: WW-2/1006781 Backup ORC: Amanda Grenier Certificate#:SI/1000089 2. Are the design, maintenance and operation of the treatment facilities adequate for the type of waste and disposal system? ❑ Yes or®No If no,please explain: Because of a history of compliance issues with this system, Arauco would like to modify the system. Currently, the facility is permitted to Pump and Haul up to 300,000 gallons of wastewater per day to Sanford's Big Buffalo W WTP because the spray field system at Arauco cannot handle the current amount of water generated. The City of Sanford is currently in the process of constructing a sewer line extension to serve Arauco. This will allow Arauco the ability to pretreat wastewater and send it directly to the Big Buffalo W WTP. The facility is permitted to operate a closed-loop recycle and conjunctive reclaimed water system. These systems have not been used in some time, however, the facility hopes to repurpose this equipment to assist in their pretreatment process. Arauco has also proposed an increase in their permitted flow limits from 56,000 gallons per day to 120,000 gallons per day. Description of existing facilities: See current permit. FORM:WQROSSR04-14 Page 1 of Proposed flow: 56,000(PPI 002) GPD Current permitted flow: 120,000 GPD Explain anything observed during the site visit that needs to be addressed by the permit,or that may be important for the permit writer to know (i.e., equipment condition, function, maintenance, a change in facility ownership, etc.). Conjunctive reclaimed Water and Closed-Loop Recycle system is not in operation. Pemuttee would like to potentially reuse these systems as part of their pretreatment system prior to sending water to Big Buffalo W WTP through planned sewer line. 3. Are the site conditions(e.g., soils, topography, depth to water table, etc.)maintained appropriately and adequately assimilating the waste? N Yes or❑No If no, please explain: Spray fields looked to be in good condition, but wet conditions have created areas of standing water in fields 2, 3, and 10.The facility is not spraying often at this time. 4. Has the site changed in any way that may affect the permit (e.g., drainage added, new wells inside the compliance boundary, new development, etc.)? ❑ Yes or N No If yes, please explain: 5. Is the residuals management plan adequate? N Yes or❑No If no,please explain: 6. Are the existing application rates(e.g.,hydraulic, nutrient) still acceptable? N Yes or❑No If no,please explain: 7. Is the existing groundwater monitoring program adequate? N Yes ❑No ❑N/A If no, explain and recommend any changes to the groundwater monitoring program: 8. Are there any setback conflicts for existing treatment, storage and disposal sites? ❑ Yes or N No If yes, attach a map showing conflict areas. 9. Is the description of the facilities as written in the existing permit correct? N Yes or❑No If no, please explain: Yes,but closed-loop and reclaimed system not in use and therefore current permit does not adequately describe the system. 10. Monitoring wells properly constructed and located? N Yes❑No ❑N/A If no,please explain: FORM:WQROSSR 04-14 Page 2 of 5 11. Are the monitoring well coordinates correct in BIMS? ® Yes❑No ❑N/A If no,' lease complete the following ex and table if necessary): Monitoring Well Latitude Longitude o r n o r o r rr o r v o r rr o r n o r rr o r rr o r e o r rr 12. Has a review of all self-monitoring data been conducted(e.g., DMR,NDMR,NDAR, GW)? ® Yes or❑No Please summarize any findings resulting from this review:The facility is compliant with effluent sampling; however fecal coliform results are very high. A review of sampling data indicates fecal numbers have decreased since the current ORC began operating the system. These high fecal counts may suggest the system is not treating the effluent to a high enough degree. This issue may be related to the age of the system. It should be noted that the current permit does not have effluent limits for any parameter. The facility frequently exceeds the permitted limit for flow at PPI 002 (Influent flow to Side D of Lagoon 5).The facility has a history of exceedances of Total Dissolved Solids and Chlorides across multiple monitoring wells. Provide input to help the permit writer evaluate any requests for reduced monitoring, if applicable. 13. Are there any permit changes needed in order to address ongoing BIMS violations? ®Yes or❑No If yes, please explain: Permittee is requesting an increase in flow limits for PPI 002.This change would likely prevent future flow violations. 14. Check all that apply: ❑No compliance issues ❑ Current enforcement action(s) ❑ Currently under JOC ®Notice(s)of violation ❑ Currently under SOC ❑ Currently under moratorium Please explain and attach any documents that may help clarify answer/comments (i.e.,NOV,NOD, etc.): See BIMS. If hard copies are needed and are not in Laserfiche, please contact Rick Trone. If the facility has had compliance problems during the permit cycle, please explain the status. Has the RO been working with the Permittee? Is a solution underway or in place? Have all compliance dates/conditions in the existing permit been satisfied? ❑ Yes ❑No ®N/A If no,please explain: 15. Are there any issues related to compliancelenforcement that should be resolved before issuing this permit? ®Yes ❑No ❑N/A If yes, please explain: History of limit exceedances for groundwater(TDS, Chlorides)and flow at PPI 002. 16. Possible toxic impacts to surface waters: 17. Pretreatment Program(POTWs only): FORM:WQROSSR 04-14 Page 3 of 5 III.REGIONAL OFFICE RECOMMENDATIONS 1.
