Your facility is currently covered for stormwater discharge under NPDES Permit NCS000201, This permit
expires on 8/31/2013. To assure consideration for continued coverage under your individual permit, you
must apply to the Division of Water Quality (DWQ) for renewal of your permit. Enclosed you will find an
individual permit Renewal Application Form, Supplemental Information request, and Stormwater Pollution
Prevention Plan Certification for your facility.
Filing the application form along with the requested supplemental information will constitute your
application for renewal of this permit. Until your permit renewal is completed and you receive a new permit,
please continue to comply with all conditions and monitoring requirements in your expired NPDES
stormwater permit.
Please comlete and return the application and supplemental information to DWQ by April 5. 2013. Failure to
request renewal within this time period may result in delay of your permit renewal. Discharge of stormwater
from your facility without coverage under a valid stormwater NPDES permit would constitute a violation of
NCGS 143-215.1 and could result in assessments of civil penalties of up to $25,000 per day.

Dear Ms. Myers:
On August 31, 2009, a compliance evaluation inspection (CEI) was conducted at the subject facility.
Your assistance was helpful and appreciated. This inspection was conducted to determine your
compliance with the subject NPDES stormwater general permits. The following observations were
noted during the inspection:
SITE DESCRIPTION
The UNC Chapel Hill Hazardous Materials Facility is the location where Hazardous Materials used by
the University of Chapel Hill are stored.
INSPECTION SUMMARY
The subject permit expires August 31, 2013.
Ms. Myers indicated that the Stormwater Pollution Prevention Plan (SP3) is currently being
updated. As an added Best Management Practice (BMP), the subject facility has added an
erosion control staff position. The Vehicle Maintenance area (service station) is full enclosed and
contains a closed -loop used oil system. A review of the outfall, records and monitoring for the
subject facility indicates full compliance with all NPDES permit requirements.

On September 13, 2007, Autumn Hoban, of the Division of Water Quality, Raleigh Regional Office, performed a
stormwater inspection of the subject facility with the assistance of Dr. Richard Miller, Environmental Affairs Manager, of
UNC Environment, Health, and Safety Department. Dr. Miller and his staff were professional, knowledgeable, and
proactive in addressing stormwater management and their facilitation of the inspection was greatly appreciated,
The facility is in compliance with the requirements of the NCS000201 NPDES Stormwater Permit issued in 1999. The
UNC-CH Hazardous Materials Facility submitted the forms and fees, as required by our agency, no later than 180 days
prior to the expiration date of the permit, which was December 31, 2004, but due to an extended processing delay at
NCDENR DWQ, the new permit has not yet been issued. The Raleigh Regional Office has followed upon the processing
delay in May 2007, and again in September 2007. We do expect the new permit to be issued in the near future.
