Dear Madam or Sir, Enclosed for your review is the 2018 Annual Report for the monitoring and reporting of the Biosolids Land Application Program for the NCDPS Odom Correctional Institution as required by 40 CFR -503 Regulations. There were no changes this past year to the Land Application program at Odom C.I. We did most of our spraying late in the season. One issue which I must bring to your attention is that I have been unable to locate the soil tests performed for 2018. I have included the soil tests for 2017 and 2016. Should 2018's be located, I will amend this report. However, I do not anticipate such an event happening. I will be certain to conduct and maintain 2019's soil test records for our next year should I still be the responsible party during that season. The NOV has been closed by DEO 's RRO. 1) Only residuals approved for this permit were applied to the permitted sites. 2) Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or the limit specified in the permit. 3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three (3) copies of laboratory results are attached. 4) Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached. 5) All other monitoring was performed in accordance with the permit and reported during the year as required and three (3) copies of certified laboratory results are attached. 6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T .1105(a) or the Pollutant Loading Rates in 15A NCAC 02T .1105(b) (applicable to 40 CFR Part 503 regulated facilities). 7) All general requirements in as specified in the Land Application Permit were complied with (applicable to 40 CFR Part 503 regulated facilities). 8) All monitoring and reporting requirements in 15A NCAC 02T .1111 were complied with (applicable to 40 CFR Part 503 regulated facilities). 9) All operations and maintenance requirements in the permit were complied with or, in the case of a deviation, prior authorization was received from the Division of Water Resources. 10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are attached to include appropriate actions and remediations. 11) Vegetative cover was maintained and proper crop management was performed on each site receing residuals, as specified in the permit. 12) No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred. 13) All buffer requirements as specified on the permit were maintained during each application of residuals. The purpose of this inspection was to ensure compliance with the subject permit. During the inspection, the knowledge and cooperation of Julio Oms was helpful and appreciated. The Odom Correctional Institution is located at 485 Odom Prison Road in Jackson, North Carolina. Class B residuals are generated at au onsite class WIN -2 wastewater treatment plant that treats domestic wastewater. Residuals generated at the facility are approved to be applied to three sites known as Tracts A -C totaling 85.10 acres. The facility is permitted to apply a tnaxintum of 7.5 dry tons per year. DPS has reviewed your notes from the site visit related to the use and condition of application tracts (A, B, and Q. At current time DPS is working with a consultant on a minor permit modification to install an access road across tract A. Included with this permit modification work is an internal evaluation to increase utilization of additional areas in the permitted tracts. DPS offers the following responses to the permit violation items identified in your letter: Condition 3 of Section M of the operating permit requires that the Raleigh Regional Office receive 24 hours' notice prior to any land application activity. Response: As identified in your inspection report, there has been unplanned staff changes at this location. Our certified operator, Geoffrey Pittard (# NC1003310) and maintenance supervisor for this facility have been reminded of this requirement. The Central Engineering (CE) department has been made aware that the notification protocol was followed with the recent application completed on September 12, 2018. Future notifications are planned to be sent via email for documentation and Incorporation into the site log book. This situation has prompted additional attention to improve DPS processes during staff turnover and changes. Included within these improvements, CE staff Is preparing a new hire/transfer of operator responsibility orientation to be signed off on by any newly designated ORC or back-up ORC. Included in this orientation will be a review of the permit and all permit conditions by the ORC and the ORC's supervisor. Condition 5 of Section f of the operating permit requires that the permittee designate and employ an ORC and back-up ORC. Response: At the time of the inspection, DPS was in negotiations to bring back Geoffrey Pittard to act as the ORC.
