Georgia-Pacific Treated Lumber, LLC (Georgia-Pacific) currently owns a treating facility in
Henderson, North Carolina. The facility is permitted to discharge storm water under NPDES
Permit No. NCS000132. Currently, Mr. Gerald McDaniel is registered as the person legally
responsible for items associated with the NPDES permit at facility. With this submittal, Georgia-
Pacific is requesting Mr. McDaniel be removed as the person legally responsible and be
replaced by Mr. Harvey Barfield. Mr. Barfield has recently assumed responsibilities as the Plant
Manager for the facility.
Attached is an original signed version of the signed Permit Owner Affiliation Designation Form
to facilitate the aforementioned updates to the facility's account.

On July 27, 2011, Thomas Ascenzo of the Raleigh Regional Office (RRO) conducted a compliance evaluation
inspection (CEI) of your lumber pressure treatment company. Your attention and assistance were appreciated, as
it helped facilitate the inspection. The Basin -Wide Information Management System (BIMS) inspection report is
attached.
The following observations were made:
Review of your Stormwater Pollution Prevention Plan Manual revealed the following:
• The plan contained a general location (USGS) map showing the outfall locations and drainage
areas.
• Narrative descriptions of practices such as best management summary, spill prevention response
plan, good housekeeping plans.
• Employee training is being conducted annually and documented as required.
• Stormwater Qualitative monitoring sheets filled out appropriately.
+ All components of the permit requirements were included in the Stormwater Manual.
2. Qualitative Monitoring conducted 7/2712010, 6/29/2011 along with Analytical Monitoring.
3. Employee Training was documented June 12, 2011, 6 out of 12 employees have been trained in the
facility, recommendation to tram all employees in spill response training.
4. Overall site is clean and well maintained with no signs of pollutants evident.
5. SP2 Manual was last updated July 2008, update and review annually as required by the permit.
6. Outfall #1 was observed to be not discharging at the time of inspection. Stream had a slight turbid
appearance to it. Cut vegetation leading to Outfall point.
7. No spills were noted at the site in the three year period. Please add a sheet to the stormwater
pollution prevention plan documenting absence of spill events.

The Division has reviewed your submittal of the permit namelownership change form for your No -Exposure
Certification, which we'received on February 9, 2007. Division personnel have reviewed and approved your
request to transfer coverage under your. Certificate of No -Exposure.
Please note that by our acceptance of your no exposure certification, you are obligated to. maintain no
exposure conditions at your facility. If conditions change such that your facility can no longer qualify for a no -
exposure exclusion, you are obligated to immediately'obtain NPDES permit coverage for your stormwater
discharge. Otherwise, the discharge becomes subject to enforcement as an un-permitted discharge. Your
conditional no -exposure exclusion expires on April 30, 2010. At that tirrle you must re -certify with the Division,
or obtain NPDES permit coverage for any stormwater discharges from your facility.
Your certification of no exposure does not affect your facility's legal requirements to obtain environmental
permits that may be required under other federal, state, or local regulations or ordinances.
