Please find enclosed the revised Certificates of Coverage for the 17 facilities that changed ownership from Concrete Supply Co to Concrete Supply Co., LLC. The terms and conditions contained in the General Permit remain unchanged and in full effect. This certificate of coverage shall become effective April 11, 2014. This Certificate of Coverage shall remain in effect for the duration of the General Permit.
