In accordance with the facility's Stormwater NPDES Permit, semi-annual monitoring must be conducted in
accordance with permit no. NCS000064. The following represent the semi-annual monitoring results from the
sampling period of January 1—June 30, 2018. Duplicate copies of the signed DMR report are attached.
The monitoring results have been compared to the benchmark values of Table 3, which are not permit limits but
are guidelines for us to use in evaluating the sources of any benchmark exceedance and for the evaluation of the
effectiveness of our BMPs.

accordance with permit no. NCS000064. The following represent the semi-annual monitoring results from the
sampling period of April 1—June 30, 2017. Duplicate copies of the signed DMR report are attached.
The monitoring results have been compared to the benchmark values of Table 3, which are not permit limits but
are guidelines for us to use in evaluating the sources of any benchmark exceedance and for the evaluation of the
effectiveness of our BMPs. One parameter of the North Outfall (TSS) appears to have had a significant amount
of suspended red clay which can elevate the TSS. A rain event delivered 1 inch within a short period of time
which created a "sheeting" action and would have scoured the parking lot, removing residual oil/grease resulting
in an increase in COD and BODS.

monitoring results from the sampling period of April 1— June 30, 2016. Duplicate copies of the

The monitoring results have been compared to the benchmark values of Table 3, which are not
permit limits but are guidelines for us to use in evaluating the sources of any benchmark
exceedance and for the evaluation of the effectiveness of our BMPs. The results from this
monitoring event are well within the benchmark values.

This report serves as the written notification of released cooling tower waters belonging to DuraFibers occurred early
morning from a valve failure associated with their cooling tower water line. It was discovered at 7:40 a.m., that due to
freezing weather, the valve failed allowing this water source to flow to the pavement below and into the north stormwater
outfall. At discovery the North outfall was diverted to wastewater treatment, eliminating any further release of this water.
The released was stopped by DuraFiber in isolating the failed valve with another valve downstream at 7:55 a.m.. Phone call
was made to NCDENR, James Moore at 3:15 p.m. same day of the release. Request to send by email, explanation of the
release to James.Moore@NCDENR.gov . Samples of this water were immediately obtained and analysis is being performed
by Celanese and outside contract laboratory. The chart below reflects the known results per the site's storm water permit.
Upon receipt of the pending parameters, an update will be forthcoming.
CNA Holdings LLC —Shelby Facility storm water contingency plan, stipulates that we maintain pumping stations at our
outfalls to ensure no waters leave the site, except approved waters as listed in our permit. In the event waters other than
approved waters leave byway of the permitted outfalls, sampling for all benchmark parameters will be conducted. TSS &
pH parameters were tested by NC Lab ##221— C N A Holdings LLC - Shelby. COD and BODS will be analyzed by outside
contract laboratory, Prism Laboratories, Inc.

Facility's. North Stormwater outfall on 1/8/2015 beginning at 1:50 p.m. These waters were release as a result of
a frozen process water line which failed at a "dead headed" location. The reason for reporting, these water left
'the site'through NCS000064 permitted outfall belonging to C N A Holdings, LLC — Shelby. The release was
reported to your voice mail the same day, 1/8/2015 at 3:15 p.m. Sample of this water was obtained and
submitted to outside certified laboratory for analysis. (See chart below)

monitoring results from the sampling period of September 1 -- November 30, 2015. Duplicate
copies of the signed DMR report are attached.
NOTE: Celanese utilizes an outside laboratory for analytical testing. In our attempt to locate
these results, Prism Laboratory discovered they had sent our results to an email not associated
with Celanese, thus the delay in submitting this semi-annual monitoring. On January 8, 4:30
p.m., we notified Bradley Bennett's, NC Stormwater Permitting Unit ; left voicemail expressing
the concern that we did not know what had happen with the results and alerted him to the fact
that the report would be mailed no later than Monday January 11. See attached email from
Prism Laboratory explaining their error. Corrective action has been implemented requiring a hard
copy to be mail to Celanese. Per Part III Section E Page 6 of 8: Reporting Requirements —
Submittals shall be received by the Division no later than 30 days from the date the facility
receives the sampling results from the laboratory.
The monitoring results have been compared to the benchmark values of Table 3, which are not
permit limits but are guidelines for us to use in evaluating the sources of any benchmark
exceedance and for the evaluation of the effectiveness of our BMPs. The results from this
monitoring event are well within the benchmark values.

conducted in accordance with permit no. NCS000064. The.following represent the semi-annual
monitoring results from the sampling period of April 1— June 30, 2015. Duplicate copies of the
signed DMR report are attached.
The monitoring results have been compared to the benchmark values of Table 3, which are not
permit limits but are guidelines for us to use in evaluating the sources of any benchmark
exceedance and for the evaluation of the effectiveness of our BMPs. The results from this
monitoring event are well within the benchmark values.

This report serves as a written notification of the release of cooling waters from Performance Fiber's operation
into Celanese — Shelby Facility's North Stormwater outfall on 8/20/14 beginning at 11:50 a.m. These waters
were release as a result of a power failure occurring with Performance Fibers utilities. The reason for reporting,
these water left the site through NCS000064 permitted outfali belonging to C N A Holdings, LLC -- Shelby. The
release was reported to you by phone today, at 2:00 p.m. (the same day). Sample of this water was obtained
and submitted to outside certified laboratory for analysis. (See chart below)
C N A Holdings, LLC— Shelby Plant's storm water contingency plan, stipulates that we maintain pumping
stations at our outfaalls to ensure no waters leave the site, except approved waters as listed in our permit,
In the event, waters other than approved waters leave by way of the permitted outfalls, sampling for all

This report serves as the final notification of the release of floor waters and condensate from a failed lift station
belonging to a resident tenant, Performance Fibers. As originally reported, these waters were discovered
leaving the site through the North Outfall on May 2, 2014 at 10:30 a.m. These waters consisted of condensate
and floor drains from their utilities department. The release was reported to you by phone on May 2, the same
day. Below are the final results from these waters.
CNA Holdings, LLC— Shelby Plant's storm water contingency plan, stipulates that we maintain pumping
stations at our outfalls to ensure no waters leave the site, except approved waters as listed in our permit.

conducted in accordance with permit no. NCS000064. The following represent the semi-annual
monitoring results from the sampling period of September 1, 2014 through November 30, 2014.
Duplicate copies of the signed DMR report are attached.
The monitoring results have been compared to the benchmark values of Table 3, which are not
permit limits but are guidelines for us to use in evaluating the sources of any benchmark
exceedance and for the evaluation of the effectiveness of our BMPs. The results from this
monitoring event are well within the benchmark values.

belonging to a resident tenant, Performance Fibers. These waters were discovered leaving the site through the
North Outfall on May 2, 2014 at 10:30 a.m. These waters consisted of condensate and floor drains from their
utilities department. The release was reported to you by phone on May 2, the same day. Sample of this water
was obtained and submitted to outside certified laboratory for analysis. (See chart below)
NOTE: waters were immediately stopped from entering the storm water distribution system, repairs made to the
lift station were completed and all water in the storm water distribution system re -directed back to the plant's
wastewater treatment.

This report serves as the final written notification of the ruptured cooling water line which occurred at 2.28 p.m.
on December 14, 2010. The rupture pipe was the result from the frigid temperatures experience during the first
part of this week. The release was reported to you by phone around 4:50 p.m. that same day. Samples of this
water were immediately obtained and analysis begun. See chart below for the updated information reflecting the
total gallons of release water from the process/potable water line which was repaired 12/16/2010 and waters
ceased leaving through the north outfall at 11:00 a.m. the same day. Also included is the additional information
on the BOD5 result for these waters leaving through the North Outfali.

at our outfalls to ensure no waters leave the site, except approved waters as listed in our permit. In the

This section of the firewater main was isolated at 7:30 a.m., stopping flow, however waters already released,

This report serves as the final report for the incident reported by telephone and mail earlier this month. CNA Holdings, Inc
— Ticona reported to you the incident whereas Invista, a leasing tenant, while performing repairs, damaged a section of the
Ticona's cooling water return line, which released waters, by way of the north stormwater outfall. Waters began leaving by
way of the north outfall at 2:10 p.m. By 3:59 p.m. repairs were completed and the incident was over. By 4:30 p.m., all
waters ceased leaving the site by way of the north outfall. Based on the duration of this incident, approximately 56,707
gallons of cooling water flowed through the north stormwater ditch leaving the site.
An examination of the drainage ditch indicated that the quantity of water released did flow to Buffalo Creek. There
were no observed environmental impacts to Buffalo Creek Samples of this water were collected and submitted to a
contract lab, Prism Laboratories, Inc. Results below:

Per telephone conversation between Mr. Mark Schroeder of CNA Holdings, Inc — Ticona and Ms. Linda Love of
Mooresville Regional DENR (July 5, 10:45 AM), I am submitting this summary of the events related to the
discharge of non -process water discharged into a stormwater ditch. This incident occurred on 7/04/02
approximately 11:00 AM due to a pump failure which allowed non -process water to exit through our north outfall
at the rate of approximately 7 gpm. On 7/05/02, 3:30 PM, repairs were completed and the stormwater
management system was operable. Throughout this event, approximately 9,000 gallons were discharged
through this ditch. An examination of the drainage ditch indicated that the quantity of water released did flow to
Buffalo Creek. There were no observed environmental impacts to Buffalo Creek.
A sample of this water was obtained at the time of discovery and was analyzed for pH (7.3), COD (<50
mg/1) and TSS ( 2 mg/l). (Results in parenthesis.) A second sample was taken 7/05/02 at 8:30 AM and
analyzed for pH (7.3), COD (44 mgll) and TSS (2 mgll). (Results in parenthesis.)

Dear Mr. Lesley:
Per my telephone conversation with you (11:17 a.m., 11/09/01), I am submitting this summary
of the. events related to the discharge of firewater into the stormwater ditches on Thursday,
11j08/01;-at the Shelby facility of CNA Holdings, Inc. This incident occurred at approximately
11:00 a.m., where a firewater line located between division valves failed releasing firewater onto
the ground, which flowed to the stormwater outfalls. The fire pumps were turned off and the
outfalls were diverted to the wastewater treatment plant (11:20 a.m.). This incident allowed for
the released of 76, 386 gallons of firewater through the outfalls. Specifically the discharge per
outfall: North outfall:3000 gallons, East outfall:1000 gallons, and South outfall:72,386 gallons.
This event was reported due to the nature of our fire water, it consist of contact/process water
which is used to cool down extruded plastic resins, is then returned and recycled. An
examination of the drainage ditches indicated that the water released through these outfalls did
flow to Buffalo Creek. There were no observed environmental impacts to Buffalo Creek.

Dear Mr. Lesley:
Per our telephone conversation (9:45 a.m., 7/11/00), I am submitting this summary of the events related to
the discharge of recycled process water into a stormwater ditch on Monday, 7/10/00, at the Shelby facility
of CNA Holdings, Inc. This incident occurred on 7/10/00 when a recycle water system filter overflowed,
releasing recycled contact cooling water to two storm drains for a short interval. This water flowed to a
sump, which overflowed to a stormwater drainage ditch. Approximately 800-900 gallons of this recycled
process water was released via this overflow; the balance was pumped from the sump to the wastewater
treatment plant.
The Shelby plant of CNA Holdings, Inc., is a manufacturing facility that converts raw materials into
different engineering polymers. The raw materials are reacted, with catalysts, to produce high viscosity
melted plastics; the plastic is then extruded, quenched by water into solid strands, and then pelletized for
future processing or shipment to customers. The water used to "quench" and solidify the polymer is
subsequently filtered, and recycled back to a storage tank for reuse in the process. The manufacturing area
for polyester contains twenty-two extrusion and quenching systems. During the process of affecting a
clean -out of the process water storage tank, the said tank taken out -of -service. In order to assure that the
demand was constant for the process water while the tank was not in use, the production personnel
inadvertently started all of the water baths simultaneously. This recycled flow exceeded the hydraulic
capacity of the filter system, resulting in the release. Because personnel were watching the equipment, the
excess flow was reduced quickly, resulting in a minimal discharge of the process water into the stormwater
system. Previous analyses of this water showed that it contains little organics. An examination of the
drainage ditch indicated that the released water went into the dry soils of the drainage ditch within 1000 ft.
of the release point; hence, none of the spilled material reached any surface water stream.
