On October 1 1, 2006, the Division of Water Quality (DWQ) received your application dated
September 27, 2006, to fill or otherwise impact 0.09 acres of wetland, 60 linear feet of perennial
stream, and 3,243 square feet of Zone 1Tar-Pamlico River basin protected riparian buffer and 2,090
square feet of Zone 2Tar-Pamlico River basin protected riparian buffer to construct the proposed
residential subdivision. On November 13, 2006, the DWQ received additional information from you,
however, the DWQ has determined that your response was incomplete as you did not address item
Nos. 2 and 4 from our previous Request For More Information correspondence dated October 20,
2006, and repeated below. The DWQ will require five copies of this additional information in order
to process your application to impact protected wetlands and/or streams on the subject property. We
will place this project on hold as incomplete until we receive this additional information. Please
provide five copies of the following information so that we may continue to review your project.
If we do not receive this additional information, your application will be withdrawn, and you will
need you will need to reapply for DWQ approval. This includes submitting a complete application
package with the appropriate fee.
Additional Information Requested:
1. Please indicate the location of the protected buffers as overlays on the site plan. Please be sure to
break out the buffers as Zone 1 and Zone 2 clearly on the site plan.
2. Please indicate the location of the grassed swale on your plan sheet. Please provide a worksheet
for the grassed Swale, and a signed and notarized Operations and Maintenance agreement.
Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and that you remain interested in continuing to pursue permitting of your project and will be
providing the DWQ the requested information at a later date. Please contact me in writing and Kyle
Barnes of the DWQ Washington Regional Office. If we do not hear from you within three weeks, we will
assume that you no longer want to pursue this project and we will consider the project as withdrawn.

or protected buffers. Please be aware that any impacts requested within your application are not
