EXTENSION APPLICATION Division of Water Resources INSTRUCTIONS FOR FORM: FTA 04-16 & SUPPORTING DOCUMENTATION This application is for sewer extensions involving gravity sewers, pump stations and force mains, or any combination that has been certified by a professional engineer and the applicant that the project meets the requirements of 15A NCAC il2'1' and the Division's Minimum Design Criteria and that plans, specifications and supporting documents have been prepared in accordance with, 15A NCAC 021, 15A NCAC 02T .0300, Dig ision policies and good engineerinY nractices. While no upfront engineering design documents are required for submittal, in accordance with ISA NCAC 02T .0305(b), design documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design calculations, and project specifications referenced in 15A NCAC 02T .0105 and the applicable minimum design criteria. These documents shall be available upon request by the Division. Projects that are deemed permitted (do not require a permit from the Division) are explained in 15A N C 02T. 303. Projects not eligible for review via the fast track process (must be submitted for full technical review): Projects that require an environmental assessment in accordance with 15A NCAC I C .0100; Projects that do not meet any part of the minimum design criteria (MDC) document; Y Projects that involve a variance from the requirements of 15A NCAC 2T; Pressure sewer systems utilizing septic tank -effluent pumps (STEPS) or simplex grinder pumps; D STEP or simplex grinder pumps connecting to pressurized systems (e.g. force mains); D Vacuum sewer systems. If so, please provide the existing permit number and issue date as noted on the application. 2 — Item V. of the application is not complete. Please provide the downstream sewer permit number, downstream sewer receiving size, and the system wide collection system permit number and owner as required. 3 — Item VI.6 is not marked. Please inform us if this should be marked as'yes', or as 'no', and we'll note the revision on the application. 4 — Item V1.7, please briefly explain how the volume of wastewater generated, 20,000 gpd, was determined. We know this was discussed in the past, but we need a written response to include with this application. 5 - There was no FTSE form submitted as required. Please submit this form. 6 — Please confirm that the pumps to be installed will be grinder pumps, which are required for FM < 4" in diameter per the Division's MDC. 7 — Since this application is >2 years old, please confirm that the force main size and length (6,020 If of 3" fm), and the pump station operational point (50 gpm), are still accurate. The narrative submitted with the application states approximately 2700 If of 3" fm will be installed. Please confirm the length of FM to be installed, and submit a revised narrative as needed. Enclosed with this letter is the letter dated February 13, 2019 from Mr. Rick Bolich of the Raleigh Regional Office discussing the terms and conditions of the moratorium removal. The letter mentions a response to a draft Special Order of Consent (SOC) document that was issued by Mr. Bolich's office recently. MGA, on behalf of the Town of Seaboard, has been actively working through the SOC process per the recommendation of the October 31 letter and will soon be finalizing this document with Mr. Bolich's office. The Town is actively working on the response to the draft SOC and will have it submitted within the next few business days. Also enclosed with this letter is an annotated version of the October 31, 2017 letter from your office recommending the Town pursue the SOC action to allow the West Fraser sewer connection to move forward. This recommendation by your office has helped lead the dialogue regarding the resolution we have developed to the issue at hand. Please provide the downstream sewer permit number, downstream sewer receiving size, and the system wide collection system permit number and owner as required. The existing Town of Seaboard system is permitted under W00003299. This Wastewater Treatment Facility is a land application site and is the end treatment facility for WIN discharged to the Facility. I have included the permit # for the Town of Seaboard WWTF in Item V. l believe this item was left blank on the original application due to the instruction that reads: RECEIVING DOWNSTREAM SEWER INFORMATION (If different than WWTF): 3 — Item VI.6 is not marked. Please inform us if this should be marked as 'yes', or as'no', and we'll note the revision on the application. This item should be marked as 'no' and I have marked this item on the included revised application. 4 — Item V1.7, please briefly explain how the volume of wastewater generated, 20,000 gpd, was determined. We know this was discussed in the past, but we need a written response to include with this application. From the SOC Application comment response letter. The flow quantities provided in the SOC application were estimated at 20,000 gallons per day. The actual flow quantities over the past several months have confirmed that the 20,000 gallons per day is an accurate amount. The flow information was provided by Rick Lynch of West Fraser. Rick Lynch is responsible for the effluent loads that are transported from West Fraser to KapStone Paper mill located in Roanoke Rapids NC. 5 - There was no FTSE form submitted as required. Please submit this form. The proposed flow request for this project involves a direct connection of force main from the proposed pump station into a gravity MH located at the existing limits of the existing gravity collection system along NC 186. This Gravity MH is part of the 10" gravity outfall that conveys wastewater to the existing bar screen in the WWTF and then to the lagoons where it is eventually land applied. 1 have filled out the FTSE form and have attached it to this email as response but am sending it to the Town's Director of Public Works for the final numbers on the Average Daily Flow for the WWTF. Sewer in the force main associated with this project does not get "repumped" before it lands in the WWTF lagoon and therefore does not obligate or consume any capacity of any pumps downstream. 6 — Please confirm that the pumps to be installed will be grinder pumps, which are required for FM < 4" in diameter per the Division's MDC. The current drawings and specifications for the project call far Submersible Grinder Pumps to be installed as detailed on sheet C 13 and as attached to this email response. 7 — Since this application is >2 years old, please confirm that the force main size and length (6,020 If of 3" fm), and the pump station operational point (50 gpm), are still accurate. The narrative submitted with the application states approximately 2700 If of 3" fm will be installed. Please confirm the length of FM to be installed, and submit a revised narrative as needed. The force main size and length shown on the application are correct at 6,020LF and 3" Diameter. The proposed pump station will have an operational point of 50gpm. The 270OLF value appears to be a typo and is being corrected in both the plans, specs and narrative. The correct length of forte main to be installed is 6,020 LF of 3" diameter force main.
