General Program information: Enclosed is a copy of the Pretreatment Program Info sheet provided by the PERCS unit. PPS, form, SNCR form, IDSF forms, and AT. As shown Beocare is SIU permit # 008 and has 1 discharge pipe. Lenoir Mirror is SIU #007 with 1 discharge pipe. These two industries discharge to the collection system that flows to the Gunpowder Creek WWTP. The analysis results of the permitted parameters do no show over allocation as per the AT provided in the HWA; permit limits should remain the same. B. General permit information for Gunpowder Creek: No new industries were added as permitted SIU's in 2018. 1. Beocare: is a medical textiles facility discharging domestic waste and process wastewaters to the collection system. The WWTP monitored the flow, analyzed for BOD, TSS, Cr, Cu, and Zn, with permit limits of BOD-400mg/I, TSS-400 mg/I, Cr- 0.400 mg/I, Cu- 0.300 mg/I, and Zn- 0.500 mg/I 2. Lenoir Mirror: is a metal finishing facility discharging domestic waste, process wastewaters and wash waters to the collection system. The WWTP monitored the flow, and analyzed for BOD, TSS, As, Cd, Cr, Cu, CN, Pb, Ag, Zn, and Total Metals which is the sum of Cu, Ni, Cr, and Zn, with permit limits of TSS- 500 mg/I, As- 0.060 mg/I, Cd- 0.010 mg/l, Cr, 0.100 mg/l, Cu-1.200 mg/I, CN- 0.020 mg/I, Pb- 0.075 mg/I, Ag- 0.075 mg/I, Zn- 0.600 mg/I and Total Metals- 2.12 mg/I. Total Toxic Organics (TTO) shall also be monitored, but a certification statement may also be used for TTO's. IU INFORMATION A. IU's: SIU —permit 007 Lenoir Mirror, and B. SIU — permit # 008 Beocare C. IU's in SNC information: 1. Beocare had issues'with Chromium the entire year of 2018. There was an NOV issued for the first 6 month period and a Public notification published in the local newspaper as well. 2. Chromium permit violations were noted for a sample collected on January 26th 2018. The Cr result for that sample was 2.16 mg/I with a permit limit of 0.4 mg/I. The industry was notified of the violation and an additional 4 samples were collected during the 1st 6 month period. 3 of the 4 additional samples were within the permit limits but the industry was still SNC 40%(2 out of 5 samples were over the permit limit) of the time with two samples being TRC (over 0.48 mg/1) as well. 3. The annual inspection for Beocare was conducted on 6/28/18 at which time I did discuss the Chromium NOV issue with the Manger of operations JR McCormick. He was going to investigate as to why the Cr results would have been above the permit limit. But he did not respond back with me. I sent numerous emails and phone calls to him with no responses. 4. For the 2nd 6 month period we collected samples at which time the Cr level was reported at 0.786. The company was notified, with no response coming from them. 5. 4 subsequent samples for Cr were obtained by October 3, 2018 with 3 of the 4 additional samples being over the permit limit. 6. The industry was notified that they were again SNC with their Cr level, and they could collect and have analyzed additional samples if they would choose to do so, to see if their additional results could pull them out of SNC. No response came from the industry. 7. Beocare has been emailed of the NOV, and a formal written NOV will be mailed to them by March 1, 2019 along with the Public Notice being published for the 2"d 6 month period 8. Tim Heim performed the annual Pretreatment inspection on January loth 2019 which included an onsite industry inspection. Beocare was chosen for the inspection. I had mentioned to Tim about the industry not corresponding with me about the ongoing Cr issue. When began the inspection Tim informed them of the critical need to communicate with the POTW of any changes and to always respond to NOV's. 9. After the visit/inspection of Becore Mr. McCormick agreed that it was his fault that the correspondence had fallen short. 10. He did get back with me soon thereafter to let me know that they had made a change to their dye vats to recycle the water so that less would be discharged to the wastewater line which he hoped would take care of the Cr levels. As shown Greer Laboratories is SIU permit #12, they have 3 discharge pipes which discharge to the collection system that flows to the Lower Creek WWTP. The analysis results of the permitted parameters do not show over -allocation as per the AT provided in the HWA; permit limitations should remain the same. B. General permit information for Lower Creek: Exela Phara has been identified as a CIU and the permitting process has begun. IU INFORMATION A. IU's: SIU —permit 12, Stallergenes Greer, pipes 1,2, & 3 B. CIU — permit (pending completion) Exela Pharma C. IU's in SNC information: Stallergenes Greer did not show SNC during the year of 2018. D.
