General Program Information
The Pretreatment Program Info Database sheet has been reviewed for any necessary
corrections. All due dates for Major Program Elements have been reviewed and are
correct. As noted on the included Pretreatment Program Info Database sheet, please
correct the following phone numbers:

No other corrections are noted for the Database at this time.
A Headworks Analysis was submitted by the City of Mebane on March 24, 2016 and
was approved by PERCS on June 3, 2016. During 2018, Long -Term Monitoring Plan
sampling continued by the POTW as outlined in the currently approved LTMP. The City
of Mebane WWTP NPDES permit is scheduled to expire on May 31, 2019. In
accordance with Part II.B.10. of this NPDES permit, a permit renewal application was
submitted to NC DEQ on November 30, 2018.
An updated Industrial Waste Survey was submitted to PERCS on January 4, 2016 and
was approved by PERCS on March 7, 2016. IWS activity continued during 2018'.
When requested, the Public Works Director provides a listing of all new sewer taps over
V (typical domestic size) that have been installed. These are reviewed for possible
new permittees. Dennis Hodge, Wastewater Director, and Amy Varinoski, Compliance
Manager, continue to serve as members of the City's Technical Review Committee.
Meetings typically occur the second Tuesday of every month and allow for the review of
plans for new construction projects and up -fit of existing facilities. Additionally, in May
2018, all existing dental facilities in Mebane were provided with a "City of Mebane 40
CFR Part 441 Fact Sheet" and a "City of Mebane One Time Compliance Report (OTCR)
for Dental Dischargers". The OTCR, per 40 CFR 441, is required to be completed and
submitted to the POTW on/before October 12, 2020, as applicable.
All SIUs were sampled and inspected in 2018. Inspection dates for each SIU are listed
below. Enforcement was issued to industries for all noted permit violations.
A Pretreatment Program Inspection was performed by Jim Gonsiewski of the Winston-
Salem Regional Office on June 26, 2018. No deficiencies were noted and overall the
inspection showed the City's pretreatment program to be satisfactory and effective.
POTW waste reduction activity is recycling. Newspapers, plastic beverage containers,
cardboard, office paper, and aluminum and steel cans are recycled by Waste Industries.
Biosolids are dewatered and used in the production of compost by a third party. EMA
Resources was the third party contracted for dewatering in 2018.

Industrial Connections & Solutions, LLC (formerly General Electric) — 6801
Industrial Drive — Permit # 0111 — 40CFR433
The facility's permit was modified effective May 15, 2018 to reflect the facility's name
change from General Electric to Industrial Connections & Solutions, LLC. Annual
inspection of the facility was conducted on November 9, 2018.
Liggett Group LLC —100 Maple Lane — Permit # 0132 — 40CFR403
The facility's SIU permit renewal, effective January 1, 2019, was transmitted to Liggett
on December 18, 2018. Annual inspection of the facility was conducted on November
7, 2018.
MetoKote Corporation —1020 Corporate Park Drive — Permit # 0133 — 40CFR433
In anticipation of the facility's permit expiration on June 30, 2019, MetoKote submitted a
permit renewal application on December 27, 2018. Annual inspection of the facility was
conducted on November 15, 2018.
Sandvik Machining Solutions US LLC (formerly Sandvik, Inc.) —1483 Dogwood
Court — Permit # 0118 — 40CFR433
Sandvik's permit was modified effective February 1, 2018 to remove the facility's self -
monitoring requirements for fluoride and selenium and effective July 1, 2018 to reflect
the facility's name change to Sandvik Machining Solutions US LLC.
Per telephone and email correspondence with Deborah Gore on November 27, 2018,
Sandvik had an exceedance of its chromium limit on October 26, 2018. However, the
cause of the exceedance was determined to be the result of an upset at the facility,
further proven by bench scale testing conducted by Sandvik in conjunction with Mrs.
Varinoski. This constituted an affirmative defense. As such, enforcement action was
not taken for this exceedance and the exceedance was not used in either Chronic or
TRC violation percentage calculations on the facility's IDSF.
Annual inspection of the facility was conducted on July 27, 2018.

Mrs. Varinoski has participated in the following training events since submittal of
the last PAR:
• NC -PC Voluntary Pretreatment Certification, Grade 3 Instructor —April
16-20, 2018
• NC AWWA-WEA Western Biological Wastewater Operators School,
Grade 3&4 Pretreatment Instructor —August 13, 2018
• NC -PC Industry Day "Pretreatment for Industrial Users", Instructor —
January 17, 2019
In addition, Amy is an active member of the North Carolina Pretreatment
Consortium, serves as a member of the Certification Board for the NC -PC
Voluntary Pretreatment Certification Program, and is a member of the NC -PC
Annual Pretreatment Conference planning committee.
Amanda Hill, the City's laboratory technician, has continued on-the-job training for
pretreatment related tasks. Amanda is also an active member of the North
Carolina Pretreatment Consortium.
