Describe the attachments:
The purpose of this correspondence is to request modifications of the Section 404 Individual Permit and
Section 401 Water Quality Certification due to an error in the calculation of the culvert length required at Site
27 (Sta. 205+35 to 211+16 -L- LT & RT). This modification request includes a revised permit drawing 47 of
83, Wetland Impact Summary sheet 83 of 83 and the DMS Acceptance Letter for 12 linear feet of additional
permanent stream impacts. Per the original authorization the mitigation ratio for this site is 1:1.
