Pretreatment Annual Report (PAR) for the period of January 1, 2018 through December 31, 2018.

NARRATIVE FOR EACH SIGNIFICANT INDUSTRIAL USER (SILT), 2018
GENERAL PROGRAM INFORMATION
The SUO, ERP, and Permits are up to date.
The LTMP was updated due to changes in the new NPDES Permit issued on
October 6, 2016. The updated LTMP was submitted to the PERCS Unit on
September 26, 2018 and an approval letter was received by the POTW on
September 27, 2018.
The updated HWA submitted on April 5, 2017 indicated an over allocation of silver
using the new WQS of 0.06 µg/L. In July 2017, POTW lowered LTMP silver PQL
to lµg/L. In June 2018, POTW started collecting extra industry samples for silver
analysis at the lowered PQL to update the AT for the HWA due January 31, 2020.
POTW also plans to use this data to review/modify current silver limits in the three
IUPs.
• The IWS was completed and submitted to the PERCS Unit on August 30, 2018 and
an approval letter was received by the POTW on September 7, 2018.
• All dates on the Division's Database Program Info Sheet are correct.
• A copy of our Program Info Sheet is enclosed and no corrections were required.
• On all three IDSFs, there are some places where there is only one data point for the
year or one for each six-month period. The reason being that the parameter is only
tested biannually or annually.
• In all cases where the tested result was BDL or less than (<), '/2 the DL was used
when averaging results.

SIU COMPLIANCE STATUS
IUP # A005: Erico Incorporated (40 CFR 433)

• The facility was inspected on November 1, 2018. The SIU received a rating of
"satisfactory". The only significant change was construction of the new
pretreatment system.
• On August 13, 2018, Erico Incorporated submitted a letter requesting an
Authorization to Construct for replacement and relocation of the MemTech
membrane filtration system beginning in October 2018. Authorization letter
granting the request was mailed on August 21, 2018. The Kontek Memkon
Microfrltration Membrane System was inspected by the POTW on November 1,
2018 and passed inspection. The Kontek system was commissioned the week of
November 5`h and began operation the week of November 12a`.
• The SIU continues to maintain a good working relationship with the POTW.
• Erico was issued one NOV for failure to submit the Industrial User Wastewater
Survey & Permit Application 180 days prior to permit expiration. Permit expired
June 30, 2018; however, Permit Application wasn't received until January 29,
2018.
• Erico's new permit was issued May 15, 2018 with an effective date of July 1, 2018
and expiration date of June 30, 2023.
IUP # A007: Pinnacle Furnishings, Inc. (40 CFR 433)
• The facility was inspected on October 9, 2018. The SIU received a rating of
"un-satisfactory" due to the following items:
■ Left front side of containment wall around chemical storage area was
damaged.
■ Several tanks were missing labels.
• January 2018 results were unable to be located.
• Follow-up inspection was conducted on November 61h. The January 2018 results
were located; however, the containment wall was still damaged and labels hadn't
been replaced.
• Second follow-up inspection was conducted on November 151h. The containment
wall was repaired and all tanks were labeled.
• The SIU has been cooperative with the POTW in resolving all issues.
• Pinnacle was issued an NOV in January 2019 for a reporting violation. The
violation was for failure to complete bi-annual self -monitoring during the July —
December 2018 period. Pinnacle had no process discharge August 2018—
December 2018; therefore, POTW declared the violation wasn't intentional and
didn't constitute SNC.

• POTW required the missed samples be collected in January 2019 and results
received by February 15, 2019. Samples were collected on January 31, 2019 and
the results were received by the POTW on February 14, 2019.
• Pinnacle's current permit expires June 30, 2020.
IUP # A008: Thermal Metal Treating, Inc. (40 CFR 433)
• The facility was inspected on October 20, 2018. The SIU received a rating of
"satisfactory". During the inspection, no significant changes were documented.
• The SIU continues to maintain a good working relationship with the POTW.
• Thermal Metal wasn't issued an NOV in 2018.
• Thermal Metal's current permit expires June 30, 2020.
