Describe the overell project in detail, including indirect impacts and the type of equipment to be used:* The project includes replacement of pavement for approbmately 6,400 LF of 4 lane and 5 lane roadway in the City of Greenville. Approxmately 1,000 LF of storm drain trunk line, which lies partially under the roadway is to be replaced. The majority of this piping exits through an existing 54-inch storm drain into the stream wfiich is the subject of this permit application. The existing pipe is rusted through at most of its corrugations in sections of this pipe. The replacement pipe is to be a 60-inch reinforced concrete pipe to eliminate the corrosion problem and to alleviate an upstream flooding problem. 4j. Comments: Keep the Pipe termination at the existing location- No lengthening except the width of a new headwall. Will provide rip rap apron to control outlet velocity. That is the reason for the length of the impact. E. Impact Justification and Mitigation 1. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: Minimize the length of impacL The width is just that required by hydraulics. 1b. Specificallydescribe measures taken to avoid or minimize the proposed impacts through construction techniques: Equipment to excavate and demo and lift pipe will be set on uplands. Stormwater Management Plan 3c. Antidegradation Calculations: Comments: This project does not create impervious surfaces. It does not increase runoff quantity or quality. All pavement replacements lie within the limits of exsting pavements. I G. Supplementary Information 1. Environmental Documentation 1a. Does the project involve an expenditure of public (federallstate/local) funds or the use of public (federallstate) land?* r Yes r No 1b. If you answered "yes" to the above, does the project require preparation of an environmental document pursuant to the requirements of the National or State (North Carolina) Environmental PolicyAct (NEPA/SEPA)?" Yes r No Comments:" As a maintenance action with project limits contained within the roadways (except this culvert construction) it does not require an environmental document. 2. Violations (DWR Requirement) 2a. Cumulative Impacts (DWR Requirement) 3a. Will this project result in additional development, which could impact nearby downstream water quality? * C' Yes r No 3b. If you answered "no," provide a short narrative description. Arlington Boulevard as it currently exists is not changed by this project. Broken pavements are replaced and drainage which currently floods withln the street rights-of-way is improved by the project. It does not open new lands to development not already served. 4. Does this project involve (1) blasting, and/or (2) other percussive activities that will be conducted by machines, such as jackhammers, mechanized pile drivers, etc.? f Yes r No Sj. What data sources did you use to determine whether your site would impact Endangered Species or Designated Critical Habitat? We are working in street and park property limits and will not be moving beyond current culvert limits except as required to install a rip rap outlet apron. Center for Biological Diversity lists 4 potential species in the area which we do not believe could be impacted by this v,nrk. The list of possible species includes: Red-Cockaded Woodpecker, Tar River Spinymussel, Atlantic Sturgeon, and Manatee. We will not impact any cavity trees, the spinymussel is rare and with highly scattered populations. USFWS says that the Spinymussel is found in high velocity v,ll oxygenated reaches of the River, and that does not match the site characteristics. Consultation Documentation Upload 6. Essential Fish Habitat (Corps Requirement) 6a. Will this project occur in or near an area designated as an Essential Fish Habitat?* C' Yes r No 6b.
