Signature Date a�/lS I Town of Forest City NPDES # NC 0025984 Narrative: During this PAR period (Jan. 1 2018 - Dec. 31 2018,1 the following summarizes the status of each Industrial User, and any changes that have occurred. IUP #0002 Eaton Corporation; No issues full compliance. Eaton's Permit was updated to lower detection limits for Cadmium, Selenium, and Silver. IUP #0004 Vallev Fine Foods VFF : Valley fine foods permit was renewed in February 2018. The permit was also modified to change Total Phosphorus to Monitor Only. VFF had one violation for Zinc and several for PH. They were fined a total of $2000.00 and surcharged $6000.00. They have installed a PH monitoring system at eff. Pipe 001 IUP #0005 WestRock: WestRock permit was renewed in February 2018. The permit was modified to change Total Phosphorus to monitor only and lower detection limits for Cadmium and Silver. WestRock had issues with Total Phosphorus and was issued several NOV's. The Total Phosphorus issue came from having to switch a chemical for cleaning the metal parts before powder coating them. It was necessary to switch chemicals for WestRock to keep a multi -million -dollar contract for Starbucks. IUP #0006 Parker Hannifin: Parker permit was renewed in February 2018. The permit was modified to lower detection limits for Cadmium. Parker had one violation for BOD and Total Phosphorus. IUP #0009 Everest Textile: Everest permit was modified to change detection limits for Cadmium, Selenium, Silver and Oil & Grease was added as monitor only. Everest had a violation for PH, and failure to sample Flow and Color. We are still in discussions with Everest on whether we can continue to receive the discharge from their DAF unit, and whether we can accept the Oil discharge form Air Cleaner. The Oil & Grease discharge form the DAF is around 2500- 3000mg/l. The Oil discharge from the Air Cleaner is a petroleum-based oil they are wanting to drip in. Forest City WWTP Forest City WWTP had no compliance issues from the violations; however, we are working hard with the industries to help correct the compliance issues. FC WWTP is still waiting to upgrade the digesters, which is scheduled for some time around August 2019. Chapter 9, PAR Guidance Pretreatment Performance Summary (PPS) 1.
