I am aware that there are significant penalties for submitting false information. • Pretreatment Coordinator Date TOWN OF YADKINVILLE NPDES # NC0020338 2018 PAR NARRATIVE GENERAL INFORMATION: PROGRAM INFORMATION: • AT, LTMP, SUO, ERP, IWS, HWA and Permits are up to date. • An updated IWS was submitted on 1/6/2017 and approved 4/7/2017. • An updated HWA was submitted on 8/18/2017 and approved on 10/31/2017. • A Pretreatment Compliance Inspection (PCI) was performed on 9/27/2017 by Jim Gonsiewski from the WSRO. • Civil Penalties collected for 2018 were $4132.20. • Permit applications have been received from Unifi Plant 2, Unifi Plant 5 and Austin Company. Renewal process to be completed post PAR completion. • Added "X" to the historical SNC report for Unifi IUP #0002 for Nickel for the first 6 month period of 2017. I was instructed to add the "X" and include a note in the narrative by Monti Hassan. PERMIT INFORMATION THE AUSTIN COMPANY IUP # 0006 - Metal Finisher SNC INFORMATION: • Austin was SNC for Cyanide for the 1st 6 month period of 2018 an NOV was issued and they were published on 8/18/2018 in the Winston Salem Journal. ORDERS AND SCHEDULE INFORMATION: None A TO C AND CONSTRUCTION INFORMATION: None MISCELLANEOUS INFORMATION: Permit application on file for renewal. • TTO Certificates statements were received for both halves of 2018. • Austin Co. permit was modified 3/1/2017. Modifications below: • 1- COD limit removed; changed to monitor only 1/month. POTW sampling 1/6 months deleted. • 2- Phenol (total) limit removed; changed to monitor only 1/6months. POTW sampling 1/6 months deleted. • Austin Co. IUP expires 4/1/2019. The Austin Co. had the following violation(s) for the January to June reporting period: • 3/20/2018- Austin Co. had a TRC Cyanide violation NOV was issued with a $500.00 fine assessed. • 1/1/2018- 6/30/2018- Austin was SNC for the 1st 6 month period due to TRC cyanide violation; Austin Co. was issued and NOV and fined for SNC and cost of publication. $500.00 fine, $192.20 publication cost. The Austin Co. had the following violation(s) for the July to December reporting period: • NONE NOVs were sent for all violations. The Austin Co. was inspected by the Town's Pretreatment Department on 11/9/18. Flow meter calibrations were performed as required by permit for 2018. B&G Foods, Snacks IUP # 0004 — Bakery SNC INFORMATION: • SIU was not in SNC this PAR year (2018) ORDERS AND SCHEDULE INFORMATION: A TO C AND CONSTRUCTION INFORMATION: None. MISCELLANEOUS INFORMATION: B&G Foods, Snacks was issued a new permit on 6/12/2015 Expiration of current permit is 5/30/20. B&G Foods, Snacks had the following violation(s) for the January to June reporting period: • January 2018- NOV issued for 1 pH violation. No fine issued. B&G Foods, Snacks had the following violations for the July to December reporting period: • NONE B&G Foods, Snacks was inspected by the Town's Pretreatment Department on 10/30/18. Flow meter calibrations were performed as required by permit for 2018. UNIFI, INC. IUP # 0002 — Textile SNC INFORMATION: None ORDERS AND SCHEDULE INFORMATION: None. A TO C AND CONSTRUCTION INFORMATION: None. MISCELLANEOUS INFORMATION: Permit application on file for renewal. Unifi IUP 0002 has had issues with monitoring equipment at their sampling manhole since it was run over by a semi truck in 2016. Unifi personnel contacted me and informed me that new equipment was to be installed in the spring of 2017. Unifi, Inc had the following violations for the January to June reporting period: • March 2018- NOV issued for 7 pH violations and 1flow violation. • April 2018- NOV issued for 5 pH violations. $100.00 fine issued. • May 2018- NOV issued for late reporting. • June 2018- NOV issued for 1- temperature, 1-pH and 6-flow violations. No data available for these parameters due to equipment issues. No fine issued. Unifi, Inc. had the following violations for the July to December reporting period: • July 2018- NOV issued for 10 pH violations. $250.00 fine issued. • August 2018- NOV issued for 5- pH violations, 1- flow violation, 1- (TRC) copper violation and 1-(TRC) zinc violation. $1150 fine issued. • November 2018- NOV issued for 4- pH violations. $140.00 issued. • November 2018- $140.00 fine due date is 1/19/2019 and will carry over to the 2019 PAR • December 2018- NOV issued for 2- pH violations. $ 100.00 fine issued.
