The structure and adjacent slopes are compromised. The proposed repair is to add Class II rip rap in the scour hole across the outlet to the extent of the wings, then grout the voids beneath the culvert, headwall, and wings. Adjacent raw sections of stream bank will be stabilized using geotextile and Class II rip rap. Sandbags for temporary dewatering will be installed in the streambed just upstream and downstream of the work site for containment to maintain a dry work area. Permanent impacts of 20LF of rip rap fill will be necessary to repair this site. Bank stabilization will be concurrent. Site 2: Recent storms have sent debris downstream and clogged the inlet of a double line RCP crossing. An end joint has separated causing the associated fill slope to fail. Additionally the adjacent ditch section approaching from the west has scoured and is unstable. The proposed repair is to reset the end joint and armor the pipe end along with armoring the ditch section. In order to maintain a dry work area, sandbags will be used to divert the stream into the eastern most pipe during the repair, therefore this stream will not be dewatered. Permanent stream impacts would be limited to SLF of bank stabilization at the tie in point of the ditch repair. Both sites are within the Tar-Pamlico River Basin, but no tree clearing or land disturbance beyond the existing transportation facility is proposed so this work is EXEMPT. No wetlands exist within either construction footprint. Following the use of the PBO for Bridge and Culvert Replacements/Repairs/Rehabilitations in Eastern North Carolina, Divisions 1-8 (June 13, 2018) the following biological conclusions are rendered: Dwarf wedgemussel No Effect, Tar River spinymussel No Effect, and Yellow Lance MA- NLAA. The nearest known occurrence reaches for either of these mussel species is over 5 miles downstream, including Atlantic pigtoe. In water work will be complete before Atlantic pigtoe is formally listed. The regularly maintained shoulders and ditch sections do not provide habitat for Michaux's sumac so a biological conclusion of No Effect is rendered. Concerning the northern long eared bat, for eastern NC (including Division 5), NCDOT has entered into a programmatic agreement with the USFWS with a Biological Conclusion of "May Affect, Likely to Adversely Affect." Since no tree clearing is proposed, no clearing reporting is required for these sites.
